Best of dermato-alergologie 2019 <\/span>
Sonia B\u0103dulici<\/span><\/p>\nLucrarea de fa\u021b\u0103 reprezint\u0103 o selec\u021bie a celor mai valoroase articole din domeniul dermato-alergologiei, publicate \u00een cursul anului precedent.
Primul dintre ele se refer\u0103 la calcipotriol, un tratament foarte utilizat \u00een psoriazis dar responsabil de irita\u021bie cutanat\u0103 [1]. Articolul demonstreaz\u0103 c\u0103 acesta poate fi \u0219i un alergen. \u0218ase pacien\u021bi sub tratament cu calcipotriol, \u00een asociere sau nu cu un dermatocorticoid, prezent\u00e2nd agravarea dermatitelor prexistente, au fost testa\u021bi cu preparatele utilizate \u0219i\/sau cu ingredientele acestora. \u00cen toate cazurile, preparatele comerciale, ca \u0219i calcipotriolul ca atare, s-au dovedit responsabile de alergie de contact; pacien\u021bii sensibiliza\u021bi la calcipotriol au putut s\u0103 tolereze, \u00eens\u0103, tratamentul topic cu al\u021bi analogi ai vitaminei D. \u00cen concluzie, atunci c\u00e2nd tratamentul topic cu calcipotriol nu reu\u0219e\u0219te s\u0103 amelioreze, sau chiar agraveaz\u0103, leziunile cutanate existente, o alergie de contact ar trebui s\u0103 fie suspectat\u0103, dar \u0219i o eventual\u0103 reactivitate \u00eencruci\u0219at\u0103.
Se pot face sau nu teste epicutanate la pacien\u021bii sub tratament cu methotrexat (MTX)? Aceasta este \u00eentrebarea la care vor r\u0103spunde autorii celui de-al doilea articol trecut \u00een revist\u0103 [2], care descriu cazul unei paciente \u00een v\u00e2rst\u0103 de 39 de ani ce primise pentru eczema sa palmo-plantar\u0103 hiperkeratozic\u0103 un tratament cu MTX, cu substitu\u021bia acidului folic. Testele epicutanate efectuate \u00een timpul acestui tratament au pus \u00een eviden\u021b\u0103 reac\u021bii intens pozitive (+++), pertinente, la sulfat de nichel \u0219i hidroperoxizi de linalool; \u00een paralel, s-a constatat o ameliorare vizibil\u0103 a leziunilor de dermatit\u0103 atopic\u0103, sub MTX, dup\u0103 un an de tratament. Concluzia acestui articol, respectiv a bilan\u021bului alergologic, a fost aceea c\u0103 MTX poate bloca doar faza de sensibilizare a hipersensibilit\u0103\u021bii de contact, reconfirm\u00e2nd existen\u021ba unor procese fiziopatologice diferite \u00een dermatita atopic\u0103 \u0219i dermatita alergic\u0103 de contact.
Proteinele de gr\u00e2u hidrolizate (PGH) pot fi responsabile de reac\u021bii de hipersensibilitate imediat\u0103 la angaja\u021bii din industria cosmeticelor. Aceasta a fost ipoteza de lucru a autorilor articolului urm\u0103tor [3], ce raporteaz\u0103 cazul unui tehnician care a prezentat dou\u0103 astfel de reac\u021bii de hipersensibilitate la interval de un an una fa\u021b\u0103 de cealalt\u0103, la cinci minute dup\u0103 ce a cur\u0103\u021bit un rezervor ce con\u021binea o pudr\u0103 de TritisolTM 12% (compus mai ales din PGH). Prick-testele pozitive pentru Tritisol \u0219i extractul de gr\u00e2u, ca \u0219i IgE-urile specifice, \u00een concentra\u021bii foarte \u00eenalte, pentru produsul cosmetic, Tritisol, PGH \u0219i gluten dezamidat, au confirmat at\u00e2t rolul PGH \u00een declan\u0219area celor dou\u0103 manifest\u0103ri de hipersensibilitate, dar \u0219i caracterul de profesionalitate al acestui caz (primul descris \u00een literatura de specialitate). Limitarea concenta\u021biei PGH \u00een cosmetice la un maximum de 1,7% ar trebui s\u0103 \u00eempiedice astfel de reac\u021bii.
Revenind la dermato-alergologia de contact, de\u0219i cromul hexavalent (bicromatul de potasiu) este considerat alergenul major al cromului, cromul trivalent poate provoca de asemenea dermatite de contact alergice, a\u0219a cum au demonstrat autorii celui de-al patrulea articol [4]. Cuantificarea cantit\u0103\u021bii de Cr(III) depus\u0103 pe tegument, ca \u0219i identificarea celei mai slabe concentra\u021bii responsabil\u0103 de o reac\u021bie vizibil\u0103, au fost efectuate pe o popula\u021bie compus\u0103 din 10 subiec\u021bi alergici la crom \u0219i 22 de martori utiliz\u00e2nd dilu\u021bii seriate de Cr(III) \u0219i de Cr(VI), ca \u0219i e\u0219antioane de piele t\u0103b\u0103cit\u0103 sau nu cu crom. Un test de extrac\u021bie a cromului, patch-teste \u00eenainte de includerea \u00een studiu \u0219i un test de utilizare (Fig. 1: a, b, c, d) au fost realizate. Doar cromul trivalent a fost eliberat din pielea t\u0103b\u0103cit\u0103 cu crom, dar acesta a provocat reac\u021bii la concentra\u021bii semnificativ mai \u00eenalte dec\u00e2t cromul hexavalent. Cum expunerea prelungit\u0103 \u0219i repetat\u0103 la pielea t\u0103b\u0103cit\u0103 cu crom a condus la o eliberare mai ales a cromului trivalent, este recomandat s\u0103 fie limitat\u0103 \u0219i eliberarea acestuia, nu doar a cromului hexavalent.
P\u0103r\u0103sind, \u00een final, domeniul alergiei cutanate, o abordare multiparametric\u0103 a irita\u021biei cutanate in vivo [5] analizeaz\u0103 efectele expunerii repetate la iritan\u021bi diferi\u021bi (sodiu lauril sulfat – SLS, hidroxid de sodiu – NaOH, n-propanol, acid acetic) asupra a 8 voluntari adul\u021bi s\u0103n\u0103to\u0219i. Referitor la eritem, pierderea de ap\u0103 transepidermic\u0103 \u0219i hidratarea cutanat\u0103, SLS a fost cauza unor diferen\u021be importante \u00een raport cu valorile de baz\u0103, nivelurile factorilor hidratan\u021bi naturali au fost semnificativ reduse de c\u0103tre SLS, NaOH \u0219i n-propanol, iar prezen\u021ba de fibre alungite \u0219i de nano-obiecte circulare (indexul de textur\u0103 dermic\u0103) a fost constatat\u0103 dup\u0103 expunerea la n-propanol \u0219i SLS (Fig. 2: A, B. C). Best of dermato-allergology 2019 <\/span>
Sonia B\u0103dulici<\/span><\/p>\nThis paper is a selection of the most valuable articles in the field of dermato-allergology published in the previous year.
The first of these concerns calcipotriol, a treatment widely used in psoriasis but responsible for cutaneous irritation [1]. The article demonstrates that it can also be an allergen. Six patients under treatment with calcipotriol, associated or not with a topical corticosteroid, who presented the worsening of pre-existing dermatitis, were tested by commercial preparations used by these ones and\/or by their ingredients. In all cases, commercial preparations, as well as calcipotriol as such, have been shown to be responsible for contact allergy; the patients sensitized to calcipotriol were able, however, to tolerate topical treatment with other vitamin D analogues. In conclusion, when topical treatment with calcipotriol fails to improve, or even aggravates, existing skin lesions, a contact allergy should be suspected but also a possible cross-reactivity.
Can epicutaneous tests be performed, or not, in patients receiving methotrexate (MTX)? This is the question at which the authors of the second review article [2] will answer; they describe the case of a 39-year-old female patient who had received for her hyperkeratotic palmoplantar eczema a treatment with MTX, with substitution of folic acid. Epicutaneous tests performed during this treatment showed extreme positive reactions (+++), clinically relevant, to both nickel sulfate and hydroperoxides of linalool; simultaneously, a visible improvement of atopic dermatitis lesions under MTX, after one year of treatment, was observed. The conclusion of this paper, namely of the allergology survey, was that MTX could block only the afferent phase of contact hypersensitivity, reconfirming the existence of different pathophysiological processes in atopic dermatitis and allergic contact dermatitis.
Hydrolyzed wheat protein (PGH) may be responsible for immediate hypersensitivity reactions in cosmetic industry employees. This was the hypothesis of the authors of the following article [3], which reports the case of a technician who had two such hypersensitivity reactions at one year from each other five minutes after cleaning a container containing a Tritisol ™ 12% powder (composed mainly of PGH). Prick-positive tests for Tritisol and wheat extract, as well as specific IgEs in very high concentrations for the cosmetic product, Tritisol, PGH and deamidated gluten, confirmed both the role of PGH in triggering the two hypersensitivity manifestations but also the character professionalism of this case (first described in the literature). Limiting the concentration of PGH in cosmetics to a maximum of 1.7% should prevent such reactions.
Returning to dermato-contact allergy, although hexavalent chromium (potassium dichromate) is considered the major allergen to chromium, trivalent chromium can also cause allergic contact dermatitis, as demonstrated by the authors of the fourth article [4]. Quantification of the amount of Cr (III) deposited on the skin, as well as identification of the weakest concentration responsible for a visible reaction, was performed on a population composed of 10 chromium allergy subjects and 22 controls using serial dilutions of Cr (III) and of Cr (VI), as well as samples of leather, whether or not chromed. A chromium extraction test, patch tests before inclusion in the study and a test of use (Fig 1: a, b, c, d) were performed. Only the trivalent chromium was released from the chrome-plated skin, but it caused reactions to significantly higher concentrations than hexavalent chromium. As prolonged and repeated exposure to chrome-plated leather has led to the release of trivalent chromium, it is recommended to limit its release, not just hexavalent chromium.
Finally, leaving the field of skin allergy, a multiparametric approach to skin irritation in vivo [5] analyzes the effects of repeated exposure to different irritants (sodium lauril sulfate – SLS, sodium hydroxide – NaOH, n-propanol, acetic acid) healthy adult volunteers. Regarding erythema, transepidermal water loss and skin hydration, SLS was the cause of significant differences with respect to baseline, levels of natural moisturizing factors were significantly reduced by SLS, NaOH and n-propanol, and the presence of elongated fibers and of circular nano-objects (dermal texture index) was found after exposure to n-propanol and SLS (Fig 2: A, B. C).
