Foliculita decalvant\u0103 este inflama\u021bie folicular\u0103 recurent\u0103 ce conduce la distruc\u021bia foliculilor pilo\u0219i, cu progersie c\u0103tre alopecie cicatricial\u0103 ireversibil\u0103. Particularitatea clinic\u0103 o reprezint\u0103 firele de p\u0103r \u00een smocuri (tufted hair), caracterizate prin emergen\u021ba unui grup de 10-25 fire de p\u0103r dintr-un singur ostium folicular. Acest aspect poate fi observat \u00een asociere \u0219i cu alte alopecii cicatriciale. Tricoscopia este o metod\u0103 auxiliar\u0103 de diagnostic, dar histopatologia determin\u0103 identificarea cauzei pentru tufted hair folliculitis.
\nPrezent\u0103m cazul a doi pacien\u021bi diagnostica\u021bi cu Foliculit\u0103 decalvant\u0103. Primul este al unui pacient \u00een v\u00e2rst\u0103 de 60 de ani, ce descrie instalarea pustulelor foliculare \u0219i a pl\u0103cii alopecice, \u00een urm\u0103 cu 4 ani, dup\u0103 un trauamtism local. Al doilea caz, este al unui pacient de 38 de ani, ce descrie apari\u021bia simptomelor \u00een urm\u0103 cu 7 ani, c\u00e2nd a fost diagnosticat cu rectocolit\u0103 ulcerohemoragic\u0103. Acesta asociaz\u0103 \u0219i Alopecie androgeno-genetic\u0103, scara III Hamilton, pentru care s-a efectuat implat de p\u0103r \u00een regiunea fronto-parietal\u0103. \u00cen regiunea parieto-occipital\u0103, pe locul alopeciei cicatriciale, s-a \u00eencercat restaurarea pl\u0103cii cu fire sintetice, cu rejec\u021bia lor. Men\u021bionez c\u0103 cele dou\u0103 cazuri se aflau la prima adresare la un consult dermatologic, f\u0103r\u0103 nici un tratament specific p\u00e2n\u0103 \u00een momentul consulta\u021biei.
\n\u00cen acest moment exist\u0103 o multitudine de alternative terapeutice, dar uneori reprezint\u0103 o adev\u0103rat\u0103 provocare alegerea tratamentului sigur \u0219i eficient. Pacien\u021bii trebuie sa \u00een\u021beleag\u0103 faptul c\u0103 rezultate acceptabile sunt ob\u021binute dup\u0103 o perioad\u0103 lung\u0103 de tratament, iar scopul este de a reduce disconfortul creat, \u00eembun\u0103t\u0103\u021birea aspectului \u0219i stoparea evolu\u021biei.<\/p>\n
Folliculitis decalvans<\/span>

The pathogenesis of Folliculitis decalvans is poorly understood, with controversial debates if the Staphylococcal infection is a primary or a secondary manifestation of the disease, or if it is a reaction of cell-mediated immunity. Folliculitis decalvans is a recurrent, purulent follicular inflammation which leads to the destruction of the hair follicle and progress to irreversible scarring alopecia. The characteristic feature is the presence of tufted hairs, groups of 10-15 hairs emerging from a single follicular opening. Tufted hair folliculitis is a clinicopathologic feature, and it can be observed in association with other scaring conditions. The use of dermatoscopy (trichoscopy) is an auxiliary tool in clinical diagnosis, but the histopathologic examination may be helpful to determine the cause of tufted hair folliculitis.<\/p>\n
We present two cases of Folliculitis decalvans. First is a case of a 60 years old male who describes the onset of the follicular pustules and the hair sheeding four years ago, related with a local trauma. The second case is a 38 years old male, who describes the appearance of frecquent outbreaks of follicular pustules and the onset of an alopecic patch seven years ago, when he was diagnosed with ulcerohemorrhagic rectocolitis.\u00a0 He also has Androgenetic alopecia type III on Hamilton scale, for which he had a hair implant in fronto-parietal region two years ago. Restoration with synthetic hairs were attempted in the parieto-occipital region, on the cicatriceal alopecia patch, which were rejected. Both patiens were at the first address to a dermatological department, without any specific treatment until the moment of the consultation.<\/p>\n
There are lots of available topical and systemic medication and sometimes it is a challenge to find an effective and safe treatment. Patiens have to understand that acceptable results are obtained with prolonged treatment and the aim is to decrease discomfort, to improve the appearance and to stop the evolution.<\/p>\n

Etiologia nu este complet elucidat\u0103, exist\u00e2nd controverse dac\u0103 infec\u021bia stafilococic\u0103 este primar\u0103 sau secundar\u0103 manifest\u0103rii acestei condi\u021bii sau este o reac\u021bie imun\u0103 mediat\u0103 celular. Foliculita decalvant\u0103 este inflama\u021bie folicular\u0103 recurent\u0103 ce conduce la distruc\u021bia foliculilor pilo\u0219i, cu progersie c\u0103tre alopecie cicatricial\u0103 ireversibil\u0103. Particularitatea clinic\u0103 o reprezint\u0103 firele de p\u0103r \u00een smocuri (tufted hair), caracterizate prin emergen\u021ba unui grup de 10-25 fire de p\u0103r dintr-un singur ostium folicular. Acest aspect poate fi observat \u00een asociere \u0219i cu alte alopecii cicatriciale. Tricoscopia este o metod\u0103 auxiliar\u0103 de diagnostic, dar histopatologia determin\u0103 identificarea cauzei pentru tufted hair folliculitis. Folliculitis decalvans<\/span> Folliculitis decalvans is a rare inflammatory scalp disorder that is classified as a neutrophilic primary cicatricial alopecia. The pathogenesis of Folliculitis decalvans is poorly understood, with controversial debates if the Staphylococcal infection is a primary or a secondary manifestation of the disease, or if it is a reaction of cell-mediated immunity. Folliculitis decalvans is a recurrent, purulent follicular inflammation which leads to the destruction of the hair follicle and progress to irreversible scarring alopecia. The characteristic feature is the presence of tufted hairs, groups of 10-15 hairs emerging from a single follicular opening. Tufted hair folliculitis is a clinicopathologic feature, and it can be observed in association with other scaring conditions. The use of dermatoscopy (trichoscopy) is an auxiliary tool in clinical diagnosis, but the histopathologic examination may be helpful to determine the cause of tufted hair folliculitis.<\/p>\n We present two cases of Folliculitis decalvans. First is a case of a 60 years old male who describes the onset of the follicular pustules and the hair sheeding four years ago, related with a local trauma. The second case is a 38 years old male, who describes the appearance of frecquent outbreaks of follicular pustules and the onset of an alopecic patch seven years ago, when he was diagnosed with ulcerohemorrhagic rectocolitis.\u00a0 He also has Androgenetic alopecia type III on Hamilton scale, for which he had a hair implant in fronto-parietal region two years ago. Restoration with synthetic hairs were attempted in the parieto-occipital region, on the cicatriceal alopecia patch, which were rejected. Both patiens were at the first address to a dermatological department, without any specific treatment until the moment of the consultation.<\/p>\n There are lots of available topical and systemic medication and sometimes it is a challenge to find an effective and safe treatment. \nPrezent\u0103m cazul a doi pacien\u021bi diagnostica\u021bi cu Foliculit\u0103 decalvant\u0103. Primul este al unui pacient \u00een v\u00e2rst\u0103 de 60 de ani, ce descrie instalarea pustulelor foliculare \u0219i a pl\u0103cii alopecice, \u00een urm\u0103 cu 4 ani, dup\u0103 un trauamtism local. Al doilea caz, este al unui pacient de 38 de ani, ce descrie apari\u021bia simptomelor \u00een urm\u0103 cu 7 ani, c\u00e2nd a fost diagnosticat cu rectocolit\u0103 ulcerohemoragic\u0103. Acesta asociaz\u0103 \u0219i Alopecie androgeno-genetic\u0103, scara III Hamilton, pentru care s-a efectuat implat de p\u0103r \u00een regiunea fronto-parietal\u0103. \u00cen regiunea parieto-occipital\u0103, pe locul alopeciei cicatriciale, s-a \u00eencercat restaurarea pl\u0103cii cu fire sintetice, cu rejec\u021bia lor. Men\u021bionez c\u0103 cele dou\u0103 cazuri se aflau la prima adresare la un consult dermatologic, f\u0103r\u0103 nici un tratament specific p\u00e2n\u0103 \u00een momentul consulta\u021biei.
\n\u00cen acest moment exist\u0103 o multitudine de alternative terapeutice, dar uneori reprezint\u0103 o adev\u0103rat\u0103 provocare alegerea tratamentului sigur \u0219i eficient. Pacien\u021bii trebuie sa \u00een\u021beleag\u0103 faptul c\u0103 rezultate acceptabile sunt ob\u021binute dup\u0103 o perioad\u0103 lung\u0103 de tratament, iar scopul este de a reduce disconfortul creat, \u00eembun\u0103t\u0103\u021birea aspectului \u0219i stoparea evolu\u021biei.<\/p>\n
