Studiul a inclus 98 de pacien\u021bi cu melanom malign, jum\u0103tate cu regresie, jum\u0103tate f\u0103r\u0103 regresie. Am analizat datele clinice \u0219i factorii prognostici histopatologici consacra\u0163i \u00een melanoamele cu regresie \u00een ariile cu \u0219i f\u0103r\u0103 regresie tumoral\u0103 \u0219i \u00een melanoamele f\u0103r\u0103 regresie. Am studiat factori de control al ciclului celular\/ oncogene: p16, p21, ciclin D1 prin teste IHC. Pozitivitatea celulelor tumorale pentru markeri de control al ciclului celular este focal\u0103 \u00een nucleii celulelor tumorale, f\u0103r\u0103 diferen\u021be semnificative \u00eentre melanoamele maligne cu regresie tumoral\u0103 \u0219i cele f\u0103r\u0103 regresie tumoral\u0103.
Am studiat fenotipul infiltratului inflamator tumoral \u00een melanomul cu \u0219i f\u0103r\u0103 regresie. Prototipul infiltratului inflamator tumoral in melanomul cutanat care are un prognostic favorabil este compus din numeroase limfocite T CD3+, putine sau absente limfocite B CD20+, putine sau absente plasmocite cu fenotip CD138+ si prezente celule Langerhans cu fenotip CD1a+ sau langerin+.
Am studiat expresia MMP 1,2,3,9,11 si 13 si TIMP 1,2 si 3 in melanoamele cu regresie in component cu regresie si fara regresie si comparativ in melanoamele fara regresie. Studiul nostru a ar\u0103tat o diminuare global\u0103 a expresiei MMP1 \u015fi MMP11 \u00een componenta nonregresata a melanoamelor cu regresie comparativ cu melanoamele fara regresie, astfel fiind probabil favorizat un comportament mai putin agresiv \u00een aceste cazuri. De asemenea, regresia \u00een melanom este corelat\u0103 cu diminuarea expresiei MMP2, MMP3 \u015fi MMP11 \u00een componenta nonregresata a melanoamelor cu regresie decat in componenta regresata a acelorasi tumori. Anumite tipuri de regresie au asociat alter\u0103ri specifice \u00een expresia MMP. Melanomul cu regresie segmentara a prezentat o expresie semnificativ mai sc\u0103zut\u0103 a MMP1 in componenta nonregresata comparativ cu melanoamele fara regresie. Melanomul cu reegresie partial\u0103 a supraexprimat MMP2 \u00een componenta nonregresata comparativ cu componenta regresata.
De asemenea, am descris diferen\u021bele \u00een expresia TIMP1, TIMP2 \u0219i TIMP3 \u00een componentele regresate \u0219i non-regresate ale melanomului cu regresie. TIMP3 a fost supraexprimat \u00een toate cazurile cu regresie segmentara \u00een componenta nonregresata \u00een compara\u021bie cu componenta regresata. Mai mult dec\u00e2t at\u00e2t, tendin\u021ba spre supraexprimarea TIMP1 \u0219i TIMP2 \u00een componenta nonregresata \u00een melanoame cu regresie partiala comparativ cu cazurile fara regresie a fost evident\u0103. Aceste rezultate sus\u021bin ipoteza c\u0103 diferen\u021bele morfologice identificate \u00een spectrul de regresie a melanomului se pot corela cu prognosticul, ceea ce ar putea explica diferentele de prognostic raportate in literatura in cazul melanoamelor cu regresie.

Histopathologic particularities of regression in melanoma<\/span>
Authors: Sabina Zurac, Mirela Cioplea, Cristiana Popp, Luciana Nichita, Carmen Dumitru, Constantin Caruntu, Daniel Boda, Carolina Constantin, Monica Neagu<\/span><\/p>\nMelanoma is a malignant tumor with increasing incidence in young patients and high mortality rate; its prognosis varies depending on the stage of the tumor – thin melanomas (pT1) are usually controlled by surgical excision within oncologic limits, whereas metastatic tumors have general survival about 1 year. Following complex interactions between tumor cells and the host immune system, melanoma may present spontaneous tumor regression; recent studies are in favor of a favorable prognostic significance of regression; in this context, we studied regression as a natural model of tumor destruction.
The study included 98 patients with malignant melanoma, half with regression, half without regression. We analyzed the clinical data and consecrated histopathological prognostic factors in melanomas with regression (in areas with and without tumor regression) and in non-regression melanomas. We studied cell cycle \/ oncogene cycle control factors: p16, p21, cyclin D1 by IHC assays. The tumor cell positive for cell cycle control markers is focal in tumor cell nuclei, with no significant difference between melanomas with regression and those without regression.
We have studied the phenotype of inflammatory tumor infiltrate in melanoma with and without regression. The prototype of inflammatory infiltrate in cutaneous melanoma that has a favorable prognosis is composed of numerous CD3 + T lymphocytes, few or absent CD20 + B lymphocytes, few or absent plasma cells with CD138 + phenotype and present Langerhans cells with CD1a + or langerin + phenotype.
We studied the MMP expression of 1,2,3,9,11 and 13 and TIMP 1,2 and 3 in melanoma with and without regression. Our study showed a global decreased of MMP1 and MMP11 expression in the non-regressed component of melanomas with regression compared to melanomas without regression, thus possibly favoring less aggressive behavior in these cases. Also, regression in melanoma is correlated with diminished expression of MMP2, MMP3 and MMP11 in the non-regressed component of melanoma with regression than in the regressed component of the same tumors. Certain types of regression have associated specific alterations in the MMP expression. Melanoma with segmental regression showes a significantly lower expression of MMP1 in the nonregressed component compared to melanoma without regression. Melanoma with partial regression has MMP2 overexpression in the non-regressed component compared to the regressed component.
We also described the differences in the expression of TIMP1, TIMP2 and TIMP3 in the regressed and non-regressed components of regression melanoma. TIMP3 was overexpressed in all cases with segmental regression in the nonregressed component compared to the regressed component. Moreover, the trend towards overexpression of TIMP1 and TIMP2 in the non-regressed component in melanoma with partial regression as compared to melanomas without regression was evident. These results support the hypothesis that the morphological differences identified in the melanoma regression spectrum may correlate with prognosis, which could explain the prognostic differences reported in the literature for regression melanomas.

Following complex interactions between tumor cells and the host immune system, melanoma may present spontaneous tumor regression; recent studies are in favor of a favorable prognostic significance of regression; in this context, we studied regression as a natural model of tumor destruction.
The study included 98 patients with malignant melanoma, half with regression, half without regression. We analyzed the clinical data and consecrated histopathological prognostic factors in melanomas with regression (in areas with and without tumor regression) and in non-regression melanomas. We studied cell cycle \/ oncogene cycle control factors: p16, p21, cyclin D1 by IHC assays. The tumor cell positive for cell cycle control markers is focal in tumor cell nuclei, with no significant difference between melanomas with regression and those without regression.
We have studied the phenotype of inflammatory tumor infiltrate in melanoma with and without regression. The prototype of inflammatory infiltrate in cutaneous melanoma that has a favorable prognosis is composed of numerous CD3 + T lymphocytes, few or absent CD20 + B lymphocytes, few or absent plasma cells with CD138 + phenotype and present Langerhans cells with CD1a + or langerin + phenotype.
We studied the MMP expression of 1,2,3,9,11 and 13 and TIMP 1,2 and 3 in melanoma with and without regression. Our study showed a global decreased of MMP1 and MMP11 expression in the non-regressed component of melanomas with regression compared to melanomas without regression, thus possibly favoring less aggressive behavior in these cases. Also, regression in melanoma is correlated with diminished expression of MMP2, MMP3 and MMP11 in the non-regressed component of melanoma with regression than in the regressed component of the same tumors. Certain types of regression have associated specific alterations in the MMP expression. Melanoma with segmental regression showes a significantly lower expression of MMP1 in the nonregressed component compared to melanoma without regression. Melanoma with partial regression has MMP2 overexpression in the non-regressed component compared to the regressed component.
We also described the differences in the expression of TIMP1, TIMP2 and TIMP3 in the regressed and non-regressed components of regression melanoma. TIMP3 was overexpressed in all cases with segmental regression in the nonregressed component compared to the regressed component. Moreover, the trend towards overexpression of TIMP1 and TIMP2 in the non-regressed component in melanoma with partial regression as compared to melanomas without regression was evident. These results support the hypothesis that the morphological differences identified in the melanoma regression spectrum may correlate with prognosis, which could explain the prognostic differences reported in the literature for regression melanomas.

Studiul a inclus 98 de pacien\u021bi cu melanom malign, jum\u0103tate cu regresie, jum\u0103tate f\u0103r\u0103 regresie. Am analizat datele clinice \u0219i factorii prognostici histopatologici consacra\u0163i \u00een melanoamele cu regresie \u00een ariile cu \u0219i f\u0103r\u0103 regresie tumoral\u0103 \u0219i \u00een melanoamele f\u0103r\u0103 regresie. Am studiat factori de control al ciclului celular\/ oncogene: p16, p21, ciclin D1 prin teste IHC. Pozitivitatea celulelor tumorale pentru markeri de control al ciclului celular este focal\u0103 \u00een nucleii celulelor tumorale, f\u0103r\u0103 diferen\u021be semnificative \u00eentre melanoamele maligne cu regresie tumoral\u0103 \u0219i cele f\u0103r\u0103 regresie tumoral\u0103.
Am studiat fenotipul infiltratului inflamator tumoral \u00een melanomul cu \u0219i f\u0103r\u0103 regresie. Prototipul infiltratului inflamator tumoral in melanomul cutanat care are un prognostic favorabil este compus din numeroase limfocite T CD3+, putine sau absente limfocite B CD20+, putine sau absente plasmocite cu fenotip CD138+ si prezente celule Langerhans cu fenotip CD1a+ sau langerin+.
Am studiat expresia MMP 1,2,3,9,11 si 13 si TIMP 1,2 si 3 in melanoamele cu regresie in component cu regresie si fara regresie si comparativ in melanoamele fara regresie. Studiul nostru a ar\u0103tat o diminuare global\u0103 a expresiei MMP1 \u015fi MMP11 \u00een componenta nonregresata a melanoamelor cu regresie comparativ cu melanoamele fara regresie, astfel fiind probabil favorizat un comportament mai putin agresiv \u00een aceste cazuri. De asemenea, regresia \u00een melanom este corelat\u0103 cu diminuarea expresiei MMP2, MMP3 \u015fi MMP11 \u00een componenta nonregresata a melanoamelor cu regresie decat in componenta regresata a acelorasi tumori. Anumite tipuri de regresie au asociat alter\u0103ri specifice \u00een expresia MMP. Melanomul cu regresie segmentara a prezentat o expresie semnificativ mai sc\u0103zut\u0103 a MMP1 in componenta nonregresata comparativ cu melanoamele fara regresie. Melanomul cu reegresie partial\u0103 a supraexprimat MMP2 \u00een componenta nonregresata comparativ cu componenta regresata.
De asemenea, am descris diferen\u021bele \u00een expresia TIMP1, TIMP2 \u0219i TIMP3 \u00een componentele regresate \u0219i non-regresate ale melanomului cu regresie. TIMP3 a fost supraexprimat \u00een toate cazurile cu regresie segmentara \u00een componenta nonregresata \u00een compara\u021bie cu componenta regresata. Mai mult dec\u00e2t at\u00e2t, tendin\u021ba spre supraexprimarea TIMP1 \u0219i TIMP2 \u00een componenta nonregresata \u00een melanoame cu regresie partiala comparativ cu cazurile fara regresie a fost evident\u0103. Aceste rezultate sus\u021bin ipoteza c\u0103 diferen\u021bele morfologice identificate \u00een spectrul de regresie a melanomului se pot corela cu prognosticul, ceea ce ar putea explica diferentele de prognostic raportate in literatura in cazul melanoamelor cu regresie.

Melanomul este o afec\u021biune tumoral\u0103 malign\u0103 cu inciden\u021b\u0103 crescut\u0103 la pacien\u021bii tineri \u0219i rat\u0103 crescut\u0103 a mortalit\u0103\u021bii, prognosticul variind \u00een func\u021bie de stadiul tumorii \u2013 melanoamele sub\u021biri pT1 sunt de regul\u0103 controlate prin excizie chirurgical\u0103 \u00een limite de siguran\u021b\u0103 oncologic\u0103, pe c\u00e2nd cele metastatice au supravie\u021buire medie de circa 1 an. \u00cen urma interactiunilor complexe dintre celulele tumorale si sistemul imun al gazdei melanomul poate prezenta regresie tumoral\u0103 spontan\u0103; studiile recente orienteaz\u0103 \u00een favoarea unei semnifica\u021bii prognostice favorabile a regresiei; \u00een acest context am studiat regresia ca model natural al distruc\u021biei tumorale.
Studiul a inclus 98 de pacien\u021bi cu melanom malign, jum\u0103tate cu regresie, jum\u0103tate f\u0103r\u0103 regresie. Am analizat datele clinice \u0219i factorii prognostici histopatologici consacra\u0163i \u00een melanoamele cu regresie \u00een ariile cu \u0219i f\u0103r\u0103 regresie tumoral\u0103 \u0219i \u00een melanoamele f\u0103r\u0103 regresie. Am studiat factori de control al ciclului celular\/ oncogene: p16, p21, ciclin D1 prin teste IHC. Pozitivitatea celulelor tumorale pentru markeri de control al ciclului celular este focal\u0103 \u00een nucleii celulelor tumorale, f\u0103r\u0103 diferen\u021be semnificative \u00eentre melanoamele maligne cu regresie tumoral\u0103 \u0219i cele f\u0103r\u0103 regresie tumoral\u0103.
Am studiat fenotipul infiltratului inflamator tumoral \u00een melanomul cu \u0219i f\u0103r\u0103 regresie. Prototipul infiltratului inflamator tumoral in melanomul cutanat care are un prognostic favorabil este compus din numeroase limfocite T CD3+, putine sau absente limfocite B CD20+, putine sau absente plasmocite cu fenotip CD138+ si prezente celule Langerhans cu fenotip CD1a+ sau langerin+.
Am studiat expresia MMP 1,2,3,9,11 si 13 si TIMP 1,2 si 3 in melanoamele cu regresie in component cu regresie si fara regresie si comparativ in melanoamele fara regresie. Studiul nostru a ar\u0103tat o diminuare global\u0103 a expresiei MMP1 \u015fi MMP11 \u00een componenta nonregresata a melanoamelor cu regresie comparativ cu melanoamele fara regresie, astfel fiind probabil favorizat un comportament mai putin agresiv \u00een aceste cazuri. De asemenea, regresia \u00een melanom este corelat\u0103 cu diminuarea expresiei MMP2, MMP3 \u015fi MMP11 \u00een componenta nonregresata a melanoamelor cu regresie decat in componenta regresata a acelorasi tumori. Anumite tipuri de regresie au asociat alter\u0103ri specifice \u00een expresia MMP. Melanomul cu regresie segmentara a prezentat o expresie semnificativ mai sc\u0103zut\u0103 a MMP1 in componenta nonregresata comparativ cu melanoamele fara regresie. Melanomul cu reegresie partial\u0103 a supraexprimat MMP2 \u00een componenta nonregresata comparativ cu componenta regresata.
De asemenea, am descris diferen\u021bele \u00een expresia TIMP1, TIMP2 \u0219i TIMP3 \u00een componentele regresate \u0219i non-regresate ale melanomului cu regresie. TIMP3 a fost supraexprimat \u00een toate cazurile cu regresie segmentara \u00een componenta nonregresata \u00een compara\u021bie cu componenta regresata. Mai mult dec\u00e2t at\u00e2t, tendin\u021ba spre supraexprimarea TIMP1 \u0219i TIMP2 \u00een componenta nonregresata \u00een melanoame cu regresie partiala comparativ cu cazurile fara regresie a fost evident\u0103. Aceste rezultate sus\u021bin ipoteza c\u0103 diferen\u021bele morfologice identificate \u00een spectrul de regresie a melanomului se pot corela cu prognosticul, ceea ce ar putea explica diferentele de prognostic raportate in literatura in cazul melanoamelor cu regresie.

Histopathologic particularities of regression in melanoma<\/span> Melanoma is a malignant tumor with increasing incidence in young patients and high mortality rate; its prognosis varies depending on the stage of the tumor – thin melanomas (pT1) are usually controlled by surgical excision within oncologic limits, whereas metastatic tumors have general survival about 1 year. Following complex interactions between tumor cells and the host immune system, melanoma may present spontaneous tumor regression; recent studies are in favor of a favorable prognostic significance of regression; in this context, we studied regression as a natural model of tumor destruction. The study included 98 patients with malignant melanoma, half with regression, half without regression. We analyzed the clinical data and consecrated histopathological prognostic factors in melanomas with regression (in areas with and without tumor regression) and in non-regression melanomas. We studied cell cycle \/ oncogene cycle control factors: p16, p21, cyclin D1 by IHC assays. The tumor cell positive for cell cycle control markers is focal in tumor cell nuclei, with no significant difference between melanomas with regression and those without regression.
We have studied the phenotype of inflammatory tumor infiltrate in melanoma with and without regression. The prototype of inflammatory infiltrate in cutaneous melanoma that has a favorable prognosis is composed of numerous CD3 + T lymphocytes, few or absent CD20 + B lymphocytes, few or absent plasma cells with CD138 + phenotype and present Langerhans cells with CD1a + or langerin + phenotype.
We studied the MMP expression of 1,2,3,9,11 and 13 and TIMP 1,2 and 3 in melanoma with and without regression. Our study showed a global decreased of MMP1 and MMP11 expression in the non-regressed component of melanomas with regression compared to melanomas without regression, thus possibly favoring less aggressive behavior in these cases. Also, regression in melanoma is correlated with diminished expression of MMP2, MMP3 and MMP11 in the non-regressed component of melanoma with regression than in the regressed component of the same tumors. Certain types of regression have associated specific alterations in the MMP expression. Melanoma with segmental regression showes a significantly lower expression of MMP1 in the nonregressed component compared to melanoma without regression. Melanoma with partial regression has MMP2 overexpression in the non-regressed component compared to the regressed component.
We also described the differences in the expression of TIMP1, TIMP2 and TIMP3 in the regressed and non-regressed components of regression melanoma. TIMP3 was overexpressed in all cases with segmental regression in the nonregressed component compared to the regressed component. Moreover, the trend towards overexpression of TIMP1 and TIMP2 in the non-regressed component in melanoma with partial regression as compared to melanomas without regression was evident. These results support the hypothesis that the morphological differences identified in the melanoma regression spectrum may correlate with prognosis, which could explain the prognostic differences reported in the literature for regression melanomas.
