Polimorfismele \u0219i muta\u021biile afect\u00e2nd glutation-S-transferazele (GST) P1, T1 \u0219i M1, precum \u015fi N-acetiltransferazele (NAT) 1 \u0219i 2 ar putea fi asociate cu un risc crescut de alergie la tiomersal. \u00cen acest context, scopul studiului a fost evaluarea frecven\u021bei sensibiliz\u0103rii la tiomersal \u00een popula\u021bia din Rom\u00e2nia, evaluarea preliminar\u0103 a rolului factorilor genetici \u00een declan\u0219area acestei sensibiliz\u0103ri, precum \u0219i identificarea medicamentelor disponibile \u00een Rom\u00e2nia care con\u021bin tiomersalul ca excipient.
\nMaterial \u0219i metode: Studiul de fa\u021b\u0103 s-a realizat pe 160 de pacien\u021bi care s-au prezentat la Spitalul Clinic Colentina cu suspiciune de reac\u021bii alergice de contact. Pacientii au fost testa\u021bi cu Bateria Standard European\u0103 \u0219i adaosurile acesteia, ce includ tiomersalul. Au fost analizate polimorfismele genelor GST (GST-P1, T1 \u0219i M1) si NAT1\/2 utiliz\u00e2nd allelic PCR \u0219i respectiv secven\u021bierea Sanger. Pentru a stabili raportul de segregare, identificarea variantelor alelice implicate \u00een producerea susceptibilit\u0103\u021bii la tiomersal s-a realizat pe probe copil-p\u0103rinte (trei paciente cu reac\u021bii pozitive la tiomersal \u00eempreun\u0103 cu unul dintre p\u0103rin\u021bi).
\nDe asemenea, compozi\u021bia tuturor produselor medicamentoase \u00eenregistrate \u00een Rom\u00e2nia a fost trecut\u0103 \u00een revist\u0103.
\nRezultate: Dintre cei 160 pacienti testa\u021bi \u00een patch au prezentat reac\u021bii pozitive la tiomersal 16 pacien\u021bi adul\u021bi (11 atopici, 5 non-atopici). Au fost observate de asemenea sensibiliz\u0103ri concomitente, in special la metale. Acestea au fost mai numeroase la pacien\u021bii atopici comparativ cu cei non-atopici.
\nReferitor la variantele genetice ale GST, la dou\u0103 familii nu au fost constatate modific\u0103ri \u00een activitatea enzimatic\u0103 a GST-M1 \u0219i GST-T1. \u00cen schimb, la o familie (mam\u0103-fiic\u0103), a fost identificat\u0103 dele\u021bia homozigot\u0103 a genei pentru glutation-S transferaza M1 (GST-M1*0\/*0) asociat\u0103 cu pierderea activit\u0103\u0163ii enzimatice corespunz\u0103toare. \u00cen aceea\u0219i familie, de\u0219i polimorfismul GST-P1 p.Ile105Val (heterozigot) a fost identificat la mam\u0103, fiica a prezentat un genotip GST-P1 p.Val105Val (homozigot). \u00cen familia reprezentat\u0103 de cuplul tat\u0103-fiic\u0103, at\u00e2t la tat\u0103 (care are vitiligo) c\u00e2t \u0219i la fiic\u0103 (care are dermatit\u0103 alergic\u0103 de contact prin sensibilizare la tiomersal \u0219i amestecul de parfumuri I) a fost prezent\u0103 varianta heterozigot\u0103 a polimorfismului GST-P1 Ile105Val. Modific\u0103ri \u00een activitatea enzimatic\u0103 a glutation-S-transferazei descrise \u00een literatur\u0103 \u00een cazul pacien\u021bilor cu vitiligo ca o predispozi\u021bie genetic\u0103 pentru aceast\u0103 afec\u021biune, ar putea fi luat\u0103 \u00een discu\u021bie pentru sensibilizarea de contact la tiomersal \u00een acest caz familial. Genotipul NAT1 identificat \u00een toate cazurile a fost genotipul wild type NAT1*4. \u00cen cazul NAT2 au fost identificate urmatoarele polimorfisme: NAT2*5 homozigot (341T>C; 481C>T), NAT2*5 – heterozigot (NAT2*5U) (282C>T; 341T>C; 481C>T; 590G>A), \u0219i respectiv NAT2*6 homozigot \/ heterozigot (282C>T; 590G>A).
\nDintre medicamentele disponibile \u00een Romania, cinci con\u021bin tiomersal, patru dintre acestea fiind medicamente oftalmologice. Sfatul pentru pacien\u021bi a fost contraindica\u021bia administr\u0103rii medicamentelor care au tiomersalul printre excipien\u021bi, precum \u0219i evitarea antiinflamatoarelor din clasa \u201eOXICAMI\u201d din cauza posibilelor reac\u021bii de hipersensibilitate \u00eencruci\u0219at\u0103 cu tiomersalul.
\nConcluzie: Frecven\u021ba reac\u021biilor pozitive la tiomersal la pacien\u021bii no\u0219tri (10%) ar justifica introducerea sa \u00een bateria standard adi\u021bional\u0103. Sunt necesare studii suplimentare care s\u0103 includ\u0103 un num\u0103r mai mare de pacienti, pentru a clarifica importan\u021ba polimorfismelor genetice \u00een alergia de contact la tiomersal.\n<\/p>\n
Genetic factors in thiomersal allergy: case studies<\/span>

B\u0103dulici, Mirela Radu, Annie Bonnevalle, Marinela Chiril\u0103<\/span><\/p>\nIntroduction: The genetic predisposition to allergic contact dermatitis is only partially known, despite numerous studies conducted over decades of research. Polymorphisms and mutations affecting glutathione-S-transferases (GST) P1, T1 and M1, as well as N-acetyltransferases (NAT) 1 and 2 could be associated with an increased risk of thiomersal allergy. In this context, the aims of the study were to evaluate the frequency of thiomersal sensitization in the Romanian population, to preliminary assess the role of genetic factors in triggering this sensitization, and to identify the medicinal products available in Romania that contain thiomersal as an excipient.
\nMaterial and methods: The present study included 160 patients who presented to the Colentina Clinical Hospital due to suspected allergic contact reactions. Patients were tested with the European Standard Battery and its additives (that include thiomersal). GST (GST-P1, T1 and M1) and NAT1 \/ 2 gene polymorphisms were analyzed using allelic PCR and Sanger sequencing, respectively. To establish the segregation ratio, the identification of allelic variants involved in the production of thiomersal susceptibility was performed using child-parent samples (three patients with positive reactions to thiomersal together with one of the parents).
\nIn addition, the composition of all medicinal products registered in Romania was reviewed.
\nResults: Out of the 160 patients tested with patch testing, 16 adult patients (11 atopic, 5 non-atopic) showed positive reactions to thiomersal. Concomitant sensitizations were also observed, especially to metals. These were more numerous in atopic patients as compared to non-atopic ones.
\nRegarding the genetic variants of GST, no changes in the enzymatic activity of GST-M1 and GST-T1 were found in two families. In contrast, in an another family (mother-daughter), homozygous deletion of the glutathione-S transferase M1 gene (GST-M1 * 0 \/ * 0) associated with loss of corresponding enzymatic activity was identified. In the same family, although the GST-P1 p.Ile105Val (heterozygous) polymorphism was identified in the mother, the daughter presented a GST-P1 p.Val105Val (homozygous) genotype. In a family represented by the father-daughter couple, both in the father (that has vitiligo) and in the daughter (that has allergic contact dermatitis by sensitization to thiomersal and fragrance mix I) the heterozygous variant of the GST-P1 Ile105Val polymorphism was present. Changes in the enzymatic activity of glutathione-S-transferase that is described in the literature in patients with vitiligo as a genetic predisposition to this condition could be considered as a possible explanation for contact sensitization to thiomersal in this familial case. The NAT1 genotype identified in all cases was the wild type NAT1 * 4 genotype. In the case of NAT2 the following polymorphisms were identified: NAT2 * 5 homozygous (341T> C; 481C> T), NAT2 * 5 – heterozygous (NAT2 * 5U) (282C> T; 341T> C; 481C> T; 590G> A) , and NAT2 * 6 homozygous \/ heterozygous (282C> T; 590G> A), respectively.
\nOf the medicinal products available in Romania, five contain thiomersal, four of which are ophthalmic drugs. The advice for patients was the contraindication of the administration of drugs containing thiomersal among excipients, as well as the avoidance of anti-inflammatory drugs from “OXICAM” class due to the possible cross-hypersensitivity reactions with thiomersal.
\nConclusion: The frequency of positive thiomersal reactions in our patients (10%) would justify its introduction into the Additional Baseline Series. Additional studies including a higher number of patients are needed to clarify the importance of genetic polymorphisms in thiomersal contact allergy.\n<\/p>\n

\nReferitor la variantele genetice ale GST, la dou\u0103 familii nu au fost constatate modific\u0103ri \u00een activitatea enzimatic\u0103 a GST-M1 \u0219i GST-T1. \u00cen schimb, la o familie (mam\u0103-fiic\u0103), a fost identificat\u0103 dele\u021bia homozigot\u0103 a genei pentru glutation-S transferaza M1 (GST-M1*0\/*0) asociat\u0103 cu pierderea activit\u0103\u0163ii enzimatice corespunz\u0103toare. \u00cen aceea\u0219i familie, de\u0219i polimorfismul GST-P1 p.Ile105Val (heterozigot) a fost identificat la mam\u0103, fiica a prezentat un genotip GST-P1 p.Val105Val (homozigot). \u00cen familia reprezentat\u0103 de cuplul tat\u0103-fiic\u0103, at\u00e2t la tat\u0103 (care are vitiligo) c\u00e2t \u0219i la fiic\u0103 (care are dermatit\u0103 alergic\u0103 de contact prin sensibilizare la tiomersal \u0219i amestecul de parfumuri I) a fost prezent\u0103 varianta heterozigot\u0103 a polimorfismului GST-P1 Ile105Val. Modific\u0103ri \u00een activitatea enzimatic\u0103 a glutation-S-transferazei descrise \u00een literatur\u0103 \u00een cazul pacien\u021bilor cu vitiligo ca o predispozi\u021bie genetic\u0103 pentru aceast\u0103 afec\u021biune, ar putea fi luat\u0103 \u00een discu\u021bie pentru sensibilizarea de contact la tiomersal \u00een acest caz familial. Genotipul NAT1 identificat \u00een toate cazurile a fost genotipul wild type NAT1*4. \u00cen cazul NAT2 au fost identificate urmatoarele polimorfisme: NAT2*5 homozigot (341T>C; 481C>T), NAT2*5 – heterozigot (NAT2*5U) (282C>T; 341T>C; 481C>T; 590G>A), \u0219i respectiv NAT2*6 homozigot \/ heterozigot (282C>T; 590G>A).
\nDintre medicamentele disponibile \u00een Romania, cinci con\u021bin tiomersal, patru dintre acestea fiind medicamente oftalmologice. Sfatul pentru pacien\u021bi a fost contraindica\u021bia administr\u0103rii medicamentelor care au tiomersalul printre excipien\u021bi, precum \u0219i evitarea antiinflamatoarelor din clasa \u201eOXICAMI\u201d din cauza posibilelor reac\u021bii de hipersensibilitate \u00eencruci\u0219at\u0103 cu tiomersalul.
\nConcluzie: Frecven\u021ba reac\u021biilor pozitive la tiomersal la pacien\u021bii no\u0219tri (10%) ar justifica introducerea sa \u00een bateria standard adi\u021bional\u0103. Sunt necesare studii suplimentare care s\u0103 includ\u0103 un num\u0103r mai mare de pacienti, pentru a clarifica importan\u021ba polimorfismelor genetice \u00een alergia de contact la tiomersal.\n<\/p>\n
Genetic factors in thiomersal allergy: case studies<\/span>

B\u0103dulici, Mirela Radu, Annie Bonnevalle, Marinela Chiril\u0103<\/span><\/p>\nIntroduction: The genetic predisposition to allergic contact dermatitis is only partially known, despite numerous studies conducted over decades of research. Polymorphisms and mutations affecting glutathione-S-transferases (GST) P1, T1 and M1, as well as N-acetyltransferases (NAT) 1 and 2 could be associated with an increased risk of thiomersal allergy. In this context, the aims of the study were to evaluate the frequency of thiomersal sensitization in the Romanian population, to preliminary assess the role of genetic factors in triggering this sensitization, and to identify the medicinal products available in Romania that contain thiomersal as an excipient.
\nMaterial and methods: The present study included 160 patients who presented to the Colentina Clinical Hospital due to suspected allergic contact reactions. Patients were tested with the European Standard Battery and its additives (that include thiomersal). GST (GST-P1, T1 and M1) and NAT1 \/ 2 gene polymorphisms were analyzed using allelic PCR and Sanger sequencing, respectively. To establish the segregation ratio, the identification of allelic variants involved in the production of thiomersal susceptibility was performed using child-parent samples (three patients with positive reactions to thiomersal together with one of the parents).
\nIn addition, the composition of all medicinal products registered in Romania was reviewed.
\nResults: Out of the 160 patients tested with patch testing, 16 adult patients (11 atopic, 5 non-atopic) showed positive reactions to thiomersal. Concomitant sensitizations were also observed, especially to metals. These were more numerous in atopic patients as compared to non-atopic ones.
\nRegarding the genetic variants of GST, no changes in the enzymatic activity of GST-M1 and GST-T1 were found in two families. In contrast, in an another family (mother-daughter), homozygous deletion of the glutathione-S transferase M1 gene (GST-M1 * 0 \/ * 0) associated with loss of corresponding enzymatic activity was identified. In the same family, although the GST-P1 p.Ile105Val (heterozygous) polymorphism was identified in the mother, the daughter presented a GST-P1 p.Val105Val (homozygous) genotype. In a family represented by the father-daughter couple, both in the father (that has vitiligo) and in the daughter (that has allergic contact dermatitis by sensitization to thiomersal and fragrance mix I) the heterozygous variant of the GST-P1 Ile105Val polymorphism was present. Changes in the enzymatic activity of glutathione-S-transferase that is described in the literature in patients with vitiligo as a genetic predisposition to this condition could be considered as a possible explanation for contact sensitization to thiomersal in this familial case. The NAT1 genotype identified in all cases was the wild type NAT1 * 4 genotype. In the case of NAT2 the following polymorphisms were identified: NAT2 * 5 homozygous (341T> C; 481C> T), NAT2 * 5 – heterozygous (NAT2 * 5U) (282C> T; 341T> C; 481C> T; 590G> A) , and NAT2 * 6 homozygous \/ heterozygous (282C> T; 590G> A), respectively.
\nOf the medicinal products available in Romania, five contain thiomersal, four of which are ophthalmic drugs. The advice for patients was the contraindication of the administration of drugs containing thiomersal among excipients, as well as the avoidance of anti-inflammatory drugs from “OXICAM” class due to the possible cross-hypersensitivity reactions with thiomersal.
\nConclusion: The frequency of positive thiomersal reactions in our patients (10%) would justify its introduction into the Additional Baseline Series. Additional studies including a higher number of patients are needed to clarify the importance of genetic polymorphisms in thiomersal contact allergy.\n<\/p>\n

\nMaterial \u0219i metode: Studiul de fa\u021b\u0103 s-a realizat pe 160 de pacien\u021bi care s-au prezentat la Spitalul Clinic Colentina cu suspiciune de reac\u021bii alergice de contact. Pacientii au fost testa\u021bi cu Bateria Standard European\u0103 \u0219i adaosurile acesteia, ce includ tiomersalul. Au fost analizate polimorfismele genelor GST (GST-P1, T1 \u0219i M1) si NAT1\/2 utiliz\u00e2nd allelic PCR \u0219i respectiv secven\u021bierea Sanger. Pentru a stabili raportul de segregare, identificarea variantelor alelice implicate \u00een producerea susceptibilit\u0103\u021bii la tiomersal s-a realizat pe probe copil-p\u0103rinte (trei paciente cu reac\u021bii pozitive la tiomersal \u00eempreun\u0103 cu unul dintre p\u0103rin\u021bi).
\nDe asemenea, compozi\u021bia tuturor produselor medicamentoase \u00eenregistrate \u00een Rom\u00e2nia a fost trecut\u0103 \u00een revist\u0103.
\nRezultate: Dintre cei 160 pacienti testa\u021bi \u00een patch au prezentat reac\u021bii pozitive la tiomersal 16 pacien\u021bi adul\u021bi (11 atopici, 5 non-atopici). Au fost observate de asemenea sensibiliz\u0103ri concomitente, in special la metale. Acestea au fost mai numeroase la pacien\u021bii atopici comparativ cu cei non-atopici.
\nReferitor la variantele genetice ale GST, la dou\u0103 familii nu au fost constatate modific\u0103ri \u00een activitatea enzimatic\u0103 a GST-M1 \u0219i GST-T1. \u00cen schimb, la o familie (mam\u0103-fiic\u0103), a fost identificat\u0103 dele\u021bia homozigot\u0103 a genei pentru glutation-S transferaza M1 (GST-M1*0\/*0) asociat\u0103 cu pierderea activit\u0103\u0163ii enzimatice corespunz\u0103toare. \u00cen aceea\u0219i familie, de\u0219i polimorfismul GST-P1 p.Ile105Val (heterozigot) a fost identificat la mam\u0103, fiica a prezentat un genotip GST-P1 p.Val105Val (homozigot). \u00cen familia reprezentat\u0103 de cuplul tat\u0103-fiic\u0103, at\u00e2t la tat\u0103 (care are vitiligo) c\u00e2t \u0219i la fiic\u0103 (care are dermatit\u0103 alergic\u0103 de contact prin sensibilizare la tiomersal \u0219i amestecul de parfumuri I) a fost prezent\u0103 varianta heterozigot\u0103 a polimorfismului GST-P1 Ile105Val. Modific\u0103ri \u00een activitatea enzimatic\u0103 a glutation-S-transferazei descrise \u00een literatur\u0103 \u00een cazul pacien\u021bilor cu vitiligo ca o predispozi\u021bie genetic\u0103 pentru aceast\u0103 afec\u021biune, ar putea fi luat\u0103 \u00een discu\u021bie pentru sensibilizarea de contact la tiomersal \u00een acest caz familial. Genotipul NAT1 identificat \u00een toate cazurile a fost genotipul wild type NAT1*4. \u00cen cazul NAT2 au fost identificate urmatoarele polimorfisme: NAT2*5 homozigot (341T>C; 481C>T), NAT2*5 – heterozigot (NAT2*5U) (282C>T; 341T>C; 481C>T; 590G>A), \u0219i respectiv NAT2*6 homozigot \/ heterozigot (282C>T; 590G>A).
\nDintre medicamentele disponibile \u00een Romania, cinci con\u021bin tiomersal, patru dintre acestea fiind medicamente oftalmologice. Sfatul pentru pacien\u021bi a fost contraindica\u021bia administr\u0103rii medicamentelor care au tiomersalul printre excipien\u021bi, precum \u0219i evitarea antiinflamatoarelor din clasa \u201eOXICAMI\u201d din cauza posibilelor reac\u021bii de hipersensibilitate \u00eencruci\u0219at\u0103 cu tiomersalul.
\nConcluzie: Frecven\u021ba reac\u021biilor pozitive la tiomersal la pacien\u021bii no\u0219tri (10%) ar justifica introducerea sa \u00een bateria standard adi\u021bional\u0103. Sunt necesare studii suplimentare care s\u0103 includ\u0103 un num\u0103r mai mare de pacienti, pentru a clarifica importan\u021ba polimorfismelor genetice \u00een alergia de contact la tiomersal.\n<\/p>\n

\nMaterial and methods: The present study included 160 patients who presented to the Colentina Clinical Hospital due to suspected allergic contact reactions. Patients were tested with the European Standard Battery and its additives (that include thiomersal). GST (GST-P1, T1 and M1) and NAT1 \/ 2 gene polymorphisms were analyzed using allelic PCR and Sanger sequencing, respectively. To establish the segregation ratio, the identification of allelic variants involved in the production of thiomersal susceptibility was performed using child-parent samples (three patients with positive reactions to thiomersal together with one of the parents).
\nIn addition, the composition of all medicinal products registered in Romania was reviewed.
\nResults: Out of the 160 patients tested with patch testing, 16 adult patients (11 atopic, 5 non-atopic) showed positive reactions to thiomersal. Concomitant sensitizations were also observed, especially to metals. These were more numerous in atopic patients as compared to non-atopic ones.
\nRegarding the genetic variants of GST, no changes in the enzymatic activity of GST-M1 and GST-T1 were found in two families. In contrast, in an another family (mother-daughter), homozygous deletion of the glutathione-S transferase M1 gene (GST-M1 * 0 \/ * 0) associated with loss of corresponding enzymatic activity was identified. In the same family, although the GST-P1 p.Ile105Val (heterozygous) polymorphism was identified in the mother, the daughter presented a GST-P1 p.Val105Val (homozygous) genotype. In a family represented by the father-daughter couple, both in the father (that has vitiligo) and in the daughter (that has allergic contact dermatitis by sensitization to thiomersal and fragrance mix I) the heterozygous variant of the GST-P1 Ile105Val polymorphism was present. Changes in the enzymatic activity of glutathione-S-transferase that is described in the literature in patients with vitiligo as a genetic predisposition to this condition could be considered as a possible explanation for contact sensitization to thiomersal in this familial case. The NAT1 genotype identified in all cases was the wild type NAT1 * 4 genotype. In the case of NAT2 the following polymorphisms were identified: NAT2 * 5 homozygous (341T> C; 481C> T), NAT2 * 5 – heterozygous (NAT2 * 5U) (282C> T; 341T> C; 481C> T; 590G> A) , and NAT2 * 6 homozygous \/ heterozygous (282C> T; 590G> A), respectively.
\nOf the medicinal products available in Romania, five contain thiomersal, four of which are ophthalmic drugs. The advice for patients was the contraindication of the administration of drugs containing thiomersal among excipients, as well as the avoidance of anti-inflammatory drugs from “OXICAM” class due to the possible cross-hypersensitivity reactions with thiomersal.
\nConclusion: The frequency of positive thiomersal reactions in our patients (10%) would justify its introduction into the Additional Baseline Series. Additional studies including a higher number of patients are needed to clarify the importance of genetic polymorphisms in thiomersal contact allergy.\n<\/p>\n
