Towards a strategic approach in spondyloarthritis

Spondyloarthritis (SpA) is the umbrella term for a broad spectrum of inflammatory rheumatic diseases, ranging from ankylosing spondylitis or radiographic axialSpA to non-radiographic axialSpA, but also psoriatic arthritis.
Over the last couple of decades, we have witnessed remarkable advances in the pathogenesis, management and treatment of SpA. The optimal management of patients with SpA requires a
combination of pharmacological and non-pharmacological treatments, including exercise and physical therapy. The advent of biologic drugs has transformed the management of patients with SpA; however, there is an unmet need for results from head-to-head studies comparing the efficacy and safety of different biologics in patients with SpA.
Despite significant advances in the management of SpA, and treatment guidelines that provide a frame work, clinicians continue to face many challenges in choosing the best treatment options for an individual patient. Strategy involves setting goals and priorities, determining actions to achieve the goals, and mobilizing resources to execute the actions. Strategy is considered a type of problem solving with three parts: a diagnosis that defines or explains the nature of the challenge, a guiding policy for dealing with the challenge and coherent actions designed to carry out the guiding policy.
An example of strategic thinking is the concept of T2T strategies, well founded in the management of other rheumatic diseases, mainly rheumatoid arthritis. The 2017 international task force update of recommendations on T2T in SpA and psoriatic arthritis advised that the treatment target should be the clinical remission or inactive disease, including musculoskeletal symptoms (axial disease, arthritis, dactylitis, enthesitis) and extra-articular manifestations.

Cătălin Codreanu este profesor de reumatologie la Universitatea de Medicină şi Farmacie “Carol Davila” din Bucureşti, precum şi managerul Centrului Clinic de Boli Reumatismale “Dr. Ion Stoia” din Bucureşti.
Dr. Codreanu a absolvit Universitatea de Medicină și Farmacie “Carol Davila” din Bucureşti, incluzând studii doctorale în reumatologie, dedicate terapiei imunomodulatoare în artrita reumatoidă. Studiile sale postdoctorale includ “Diplome d’Universite d’Immunopathologie Articulaire”, obţinută la Universitatea din Montpellier (Franţa). Dr. Codreanu a obţinut titlul de Master în managementul serviciilor de sănătate, la Academia de Studii Economice din Bucureşti.
Domeniile principale de interes includ tratamentul artritei reumatoide, în mod particular cu methotrexat şi agenţi biologici, artrita precoce, precum şi supravegherea evoluţiei bolilor reumatice prin utilizarea registrelor de boală. Dr. Codreanu este membru în mai multe societăţi ştiinţifice naţionale şi internaţionale: îndeplineşte funcţia de Preşedinte al Societăţii Române de Reumatologie şi Preşedinte al Registrului Român de Boli Reumatice.
