Psoriazisul vulgar este o boal\u0103 inflamatorie cronic\u0103 a pielii, mediat\u0103 de limfocitele T helper, ce are o prevalen\u021b\u0103 de aproximativ 1% \u00een cazul copiilor. Psoriazisul pediatric se \u00eemparte \u00een 3 grupuri distincte: psoriazis infantil, psoriazis cu debut precoce \u0219i psoriazis pediatric asociat cu artropatie psoriazic\u0103. Napkin psoriazis reprezint\u0103 un subtip al psoriazisului infantil care afecteaz\u0103 \u00een principal copiii cu v\u00e2rsta sub 2 ani.
Prezent\u0103m cazul unui sugar de 2 luni, n\u0103scut la termen, \u00een mod natural, cu greutatea \u0219i \u00een\u0103l\u021bimea \u00een limite normale, care este adus \u00een contextul prezen\u021bei unor placi \u0219i placarde eritemato-scuamoase \u00een zona anogenital\u0103, regiunea gluteal\u0103, pe membrele superioare, inferioare, pe abdomen \u0219i la nivelul fe\u021bei.
Declarativ, debutul leziunilor cutanate a fost \u00een prima lun\u0103 de via\u021b\u0103, la c\u00e2teva zile dup\u0103 o infec\u021bie a tractului respirator superior, ini\u021bial la nivelul fe\u021bei \u0219i \u00een zona scutecului, cu extinderea ulterioar\u0103 a leziunilor.
Pe de o parte, din cauza v\u00e2rstei fragede \u0219i a st\u0103rii alterate a pacientului, examenul histopatologic pentru confirmare nu s-a putut realiza. Pe de alt\u0103 parte, prezen\u021ba de leziuni tipice \u00een alte arii anatomice, semnul sperman\u021betului \u0219i semnul Auspitz pozitive la gratajul metodic Brocq, aspectul dermatoscopic cu vase punctiforme distribuite regulat pe fond eritematos sunt argumente \u00een favoarea diagnosticului de napkin psoriasis.
Tratamentul topic a constat \u00een \u00eendep\u0103rtarea factorilor predispozan\u021bi \u0219i a iritan\u021bilor (scutec, urin\u0103) prin igien\u0103 riguroas\u0103 cu s\u0103pun dermatologic, aplicarea dermatocorticosteroizilor non-fluorina\u021bi topici, utilizarea emolientelor, terapia cu lumina polarizat\u0103, toate acestea urmate de evolu\u021bia favorabil\u0103 a leziunilor.
Napkin psoriasis este o form\u0103 relativ rar\u0103 de psoriazis infantil. Acesta pune probleme de diagnostic diferen\u021bial cu dermatita iritativ\u0103 de contact \u0219i intertrigo-ul levuric. Considerat de noi a fi o \u201cpovar\u0103 ce apas\u0103 pe umeri mult prea fragili\u201d, tratamentul trebuie abordat cu precau\u021bie, av\u00e2nd \u00een vedere particularit\u0103\u021bile morfofunc\u021bionale ale pielii sugarului. Ace\u0219ti mici pacien\u021bi trebuie urm\u0103ri\u021bi cu mare aten\u021bie, iar p\u0103rin\u021bii trebuie informa\u021bi despre aceast\u0103 boal\u0103 \u0219i posibilit\u0103\u021bile ei de evolu\u021bie.<\/p>\n

Psoriasis vulgaris is a chronic inflammatory disease of the skin, mediated by T helper lymphocytes, which has a prevalence of 1% in children. Pediatric psoriasis is divided into 3 distinct groups: infantile psoriasis, early onset psoriasis and pediatric psoriasis associated with psoriatic arthropathy. Napkin psoriasis is a subtype of infantile psoriasis that mainly affects children under the age of 2.
We present the case of a 2-month-old infant, born at term, naturally, with normal weight and height, who was brought in our clinic for the presence of some erythematous-squamous plaques and placards located in the anogenital area, gluteal region, on the upper, lower limbs, on the abdomen and face.
According to the family, the onset of the skin lesions was in the first month of life, a few days after an infection of the upper respiratory tract, initially affecting the face and the diaper area, with subsequent extension. On the one hand, due to the young age of the patient, the histopathological confirmation could not be performed. On the other hand, the presence of typical lesions in other anatomical areas, the positive Auspitz sign, the characteristic dermatoscopic appearance with pointy little vessels regularly distributed on erythematous background are all arguments for the diagnosis of napkin psoriasis. Topical treatment consisted in removing predisposing factors and irritants (diaper, urine) through rigorous hygiene with dermatological soap, use of topical non-fluorinated dermatocorticosteroids, use of emollients, polarized light therapy, all followed by favorable evolution of the lesions. Napkin psoriasis is a rare form of infantile psoriasis. The differential diagnosis includes irritant contact dermatitis and intertrigo. Considered by us a “burden that presses on the fragile shoulders of the new-born”, the treatment of this problematic disease must be approached with caution, especially because of specific morphofunctional features of the baby’s skin. These small patients need special care and parents must be informed about this disease and its possibilities of evolution.<\/p>\n
