SARS-CoV-2 folose\u0219te enzima de conversie a angiotensinei 2 (ACE2) pentru a intra \u00een celulele umane; celulele endoteliale exprim\u0103 niveluri ridicate de ACE2 fiind supuse unor efecte citopatice virale directe cu leziuni severe cu disruptive de membrane celulare. Ca rezultate directe, au fost descrise mai multe tipuri de leziuni vasculare: tromboz\u0103 multiorgan\u0103 extins\u0103 (\u00een capilare, arteriole \u0219i venule dar \u0219i \u00een vase mai mari cu thromboembolism pulmonar ulterior fatal), microangiopatie, activare endotelial\u0103 \u0219i angiogenez\u0103 extins\u0103.
Am analizat leziunile vasculare \u00een 10 autopsii (grupul de studiu A) \u0219i 21 de biopsii cutanate \u0219i gastro-intestinale efectuate la pacien\u021bii cu COVID-19 (grupul de studiu B); folosim ca martor 10 autopsii (grupul de control A) efectuate la pacien\u021bii deceda\u021bi cu gripa A H1N1 \u0219i 21 de biopsii cutanate \u0219i gastro-intestinale efectuate la pacien\u021bii cu dermatoze inflamatorii \u0219i inflama\u021bii nespecifice gastro-intestinale asortate dup\u0103 sex \u0219i v\u00e2rst\u0103 (grupul de control B).

SARS-CoV-2 uses the angiotensin converting enzyme 2 (ACE2) to entry into human cells; endothelial cells express high levels of ACE2 being subject of direct viral cytopathic effects with severe injury with disrupted cell membranes. As direct results, several types of vascular lesions were described: extended multiorgan thrombosis (in capillaries, arterioles and venules but also in larger vessels with subsequent fatal pulmonary thromboembolisms), microangiopathy, endothelial activation and extensive angiogenesis.
We analyzed the vascular lesions in 10 autopsies (study group A); and 21 cutaneous and gastrointestinal (GI) tract biopsies performed on patients with COVID-19 (study group B); we use as control 10 autopsies (control group A) performed on A H1N1 influenza deceased patients and 21 cutaneous and GI tract biopsies performed on patients with inflammatory dermatoses and nonspecific inflammation of GI tract matched by sex and age (control group B).
All patients in study group A presented fibrin microthrombosis in multiple organs; 6 patients presented thrombi in large vessels; 7 patients presented recent infarctions; 8 patients present microangiopathy in study group A; no thrombosis or microangiopathy were present in control group A. 11 patients with fibrin microthrombi and 4 cases with microangiopathy were described in study group B while none of the patients in control group B presented such changes.
In our experience, COVID-19 presents multiple vascular lesions; understanding the cascade of key mediators involved in these processes open the gate towards targeted therapy of COVID-19 patients with severe forms of disease in order to reduce morbidity and mortality.<\/p>\n

Am analizat leziunile vasculare \u00een 10 autopsii (grupul de studiu A) \u0219i 21 de biopsii cutanate \u0219i gastro-intestinale efectuate la pacien\u021bii cu COVID-19 (grupul de studiu B); folosim ca martor 10 autopsii (grupul de control A) efectuate la pacien\u021bii deceda\u021bi cu gripa A H1N1 \u0219i 21 de biopsii cutanate \u0219i gastro-intestinale efectuate la pacien\u021bii cu dermatoze inflamatorii \u0219i inflama\u021bii nespecifice gastro-intestinale asortate dup\u0103 sex \u0219i v\u00e2rst\u0103 (grupul de control B).
\u00cen grupul de studiu A to\u021bi pacien\u021bii au prezentat microtrombi fibrino\u0219i \u00een multiple organe, 6 pacien\u021bi au prezentat trombi \u00een vase mari, 7 pacien\u021bi au prezentat infarcte recente; 8 pacien\u021bi prezint\u0103 microangiopatie; \u00een grupul de control A, nu au fost prezente tromboze sau microangiopatii. 11 pacien\u021bi cu microtrombi fibrino\u0219i \u0219i 4 cazuri cu microangiopatie au fost descri\u0219i \u00een grupul de studiu B; niciunul dintre pacien\u021bii din grupul de control B nu a prezentat astfel de modific\u0103ri.

SARS-CoV-2 uses the angiotensin converting enzyme 2 (ACE2) to entry into human cells; endothelial cells express high levels of ACE2 being subject of direct viral cytopathic effects with severe injury with disrupted cell membranes. As direct results, several types of vascular lesions were described: extended multiorgan thrombosis (in capillaries, arterioles and venules but also in larger vessels with subsequent fatal pulmonary thromboembolisms), microangiopathy, endothelial activation and extensive angiogenesis.
We analyzed the vascular lesions in 10 autopsies (study group A); and 21 cutaneous and gastrointestinal (GI) tract biopsies performed on patients with COVID-19 (study group B); we use as control 10 autopsies (control group A) performed on A H1N1 influenza deceased patients and 21 cutaneous and GI tract biopsies performed on patients with inflammatory dermatoses and nonspecific inflammation of GI tract matched by sex and age (control group B).
All patients in study group A presented fibrin microthrombosis in multiple organs; 6 patients presented thrombi in large vessels; 7 patients presented recent infarctions; 8 patients present microangiopathy in study group A; no thrombosis or microangiopathy were present in control group A. 11 patients with fibrin microthrombi and 4 cases with microangiopathy were described in study group B while none of the patients in control group B presented such changes.
In our experience, COVID-19 presents multiple vascular lesions; understanding the cascade of key mediators involved in these processes open the gate towards targeted therapy of COVID-19 patients with severe forms of disease in order to reduce morbidity and mortality.<\/p>\n

We analyzed the vascular lesions in 10 autopsies (study group A); and 21 cutaneous and gastrointestinal (GI) tract biopsies performed on patients with COVID-19 (study group B); we use as control 10 autopsies (control group A) performed on A H1N1 influenza deceased patients and 21 cutaneous and GI tract biopsies performed on patients with inflammatory dermatoses and nonspecific inflammation of GI tract matched by sex and age (control group B).
All patients in study group A presented fibrin microthrombosis in multiple organs; 6 patients presented thrombi in large vessels; 7 patients presented recent infarctions; 8 patients present microangiopathy in study group A; no thrombosis or microangiopathy were present in control group A. 11 patients with fibrin microthrombi and 4 cases with microangiopathy were described in study group B while none of the patients in control group B presented such changes.
In our experience, COVID-19 presents multiple vascular lesions; understanding the cascade of key mediators involved in these processes open the gate towards targeted therapy of COVID-19 patients with severe forms of disease in order to reduce morbidity and mortality.<\/p>\n
