Cuvinte cheie: dermatita atopic\u0103, alergii, atopie, mar\u0219ul alergic<\/span><\/p>\n Dermatita atopic\u0103 constituie o entitate clinic\u0103 ce traduce o suferin\u021b\u0103 cutanat\u0103 persistent\u0103 cu exacerb\u0103ri repetate, exprimat\u0103 prin prurit, leziuni tegumentare cu morfologie \u0219i distribu\u021bie tipic\u0103 asociate unui istoric personal sau familial de atopie. Pacien\u021bii cu dermatita atopic\u0103 prezint\u0103 o rat\u0103 mai mare a afec\u021biunilor alergice dec\u00e2t popula\u021bia general\u0103. Un procent semnificativ al copiilor cu dermatit\u0103 atopic\u0103 va dezvolta \u00een adolescen\u021b\u0103 sau la maturitate rinita alergic\u0103 sau astm bron\u0219ic, progresie ce poart\u0103 denumirea de \u201cMar\u0219 alergic\u201d. O propor\u021bie de 10% – 20% dintre pacien\u021bii cu dermatita atopic\u0103 sufer\u0103 de alergii alimentare ce au ca expresie clinic\u0103 urticaria sau anafilaxia fa\u021b\u0103 de popula\u021bia general\u0103 \u00een care procentul acestora este de 1% – 3%. La sugari \u0219i copii mici (<6 ani) cu dermatita atopic\u0103 procentul alergiilor alimentare ce pot conduce la exacerb\u0103ri variaz\u0103 \u00eentre 15% – 30%. Debutul precoce al dermatitei atopice \u0219i formele clinice severe sunt frecvent asociate cu alergiile alimentare IgE mediate induse de proteinele din ou, lapte \u0219i arahide. Pacien\u021bii ce asociaz\u0103 sensibiliz\u0103ri alimentare \u0219i respiratorii induse de acarieni prezint\u0103 risc crescut de persisten\u021b\u0103 a dermatitei \u0219i dup\u0103 v\u00e2rsta de 5 ani. La adul\u021bii cu dermatita atopic\u0103 \u00eent\u00e2lnim dou\u0103 aspecte: continuarea dermatitei cu debut \u00een copil\u0103rie \u0219i debutul tardiv instalat la maturitate. Asocierea sensibiliz\u0103rii alergice de contact trebuie suspectat\u0103 \u00een cazurile de dermatita atopic\u0103 moderat-severe refractare la tratament \u0219i cu dispunere atipic\u0103 a leziunilor. Determin\u0103rile moleculare singleplex sau multiplex, testul de activarea al bazofilelor, testele epicutanate patch.<\/span><\/p>\n Atopic dermatitis and allergies<\/span> Keywords: atopic dermatitis, allergies, atopy, atopic march<\/span><\/p>\n Atopic dermatitis is a clinical entity that translates a persistent skin condition with repeated exacerbations, expressed by pruritus, skin lesions with morphology and typical distribution and the association of a personal or family history of atopy. Patients with atopic dermatitis have a higher rate of allergic diseases than the general population. A significant percentage of children with atopic dermatitis will develop allergic rhinitis or asthma in adolescence or adulthood, progression called “Atopic march”. A proportion of 10-20% of patients with atopic dermatitis suffer from food allergies whose clinical expression is urticaria or anaphylaxis compared to the general population in which their percentage is 1-3%. In infants and young children (<6 years) with atopic dermatitis, the percentage of food allergies that can lead to exacerbations varies between 15-30%. Early onset of atopic dermatitis and severe clinical forms are most often associated with IgE-mediated food allergies induced by egg, milk and peanut proteins. Patients with atopic dermatitis associated with food and respiratory sensitization induced by mites have an increased risk of persistent dermatitis even after the age of 5 years. Adult atopic dermatitis can be differentiated into two main disease courses: the continuation of dermatitis with onset in childhood and late onset dermatitis in adulthood. Allergic contact dermatitis should be suspected in cases of moderate to severe atopic dermatitis refractory to treatment and with atypical disposition of the lesions.
