Lichenul scleros este o dermatoz\u0103 inflamatorie autoimun\u0103, cronic\u0103, scleroatrofic\u0103, ce afecteaz\u0103 regiunea anogenital\u0103 \u00een procent de peste 85% din cazuri \u0219i \u00eentr-o mic\u0103 m\u0103sur\u0103 zonele extragenitale (15%). Este o boal\u0103 relativ rar\u0103, mai frecvent \u00eent\u00e2lnit\u0103 la femei de c\u00e2t la b\u0103rba\u021bi F:B=5:1, put\u00e2nd afecta inclusiv copiii. Etiologia bolii r\u0103m\u00e2ne necunoscut\u0103, fiind incomplet elucidat\u0103. Sunt incrimina\u021bi factori genetici, autoimuni, infec\u021bio\u0219i, hormonali. Cea mai grav\u0103 complica\u021bie este reprezentat\u0103 de apari\u021bia carcinomului scuamocelular ce poate apare la aproximativ 10 % din b\u0103rba\u021bi. Sunt propuse mai multe alternative terapeutice pentru leziunile genitale, cea mai utilizat\u0103 fiind corticoterapia local\u0103. Prezent\u0103m cazul unui pacient HP, \u00een v\u00e2rst\u0103 de 42 ani, ce se adreseaz\u0103 pentru apari\u021bia de aproximativ 8 luni a unor pl\u0103ci u\u0219or indurate, albicioase, cu zone pigmentate, la nivel penian, asociate cu usturime local\u0103, durere la contact sexual, prurit. Pacientul asociaz\u0103 de 6 ani un diabet zaharat tip 2, cu evolu\u021bie ondulant\u0103 \u0219i de 5 ani tiroidit\u0103 autoimun\u0103. Examenul clinic a fost completat cu dermatoscopie \u0219i ex histopatologic care a transat diagnosticul de lichen scleros. Evolu\u021bia a fost favorabil\u0103 sub tratamentul cu corticosteroizi topici poten\u021bi.

Sclerotic lichen is an autoimmune, chronic, scleroatrophic inflammatory dermatosis that affects the anogenital region in over 85% of cases and to a lesser extent the extragenital areas (15%). It is a relatively rare disease, more common in women than in men; F:B = 5: 1, and can even affect children. The etiology of the disease remains unknown, being incompletely elucidated. Genetic, autoimmune, infectious, hormonal factors are incriminated. The most serious complication is the appearance of squamous cell carcinoma that can occur in about 10% of men. Several therapeutic alternatives are proposed for genital lesions, the most used being local corticotherapy. We present the case of a patient, HP, aged 42 years, who addresses for the presence of approximately 8 months slightly hardened, whitish plaques, with pigmented areas, at the penile level, associated with local stinging, pain in sexual contact, pruritus. The patient has been associated with type 2 diabetes for 6 years, with undulating evolution and autoimmune thyroiditis for 5 years. The clinical examination was supplemented with dermatoscopy and histopathological examination that diagnosed lichen sclerosis. The evolution was favorable under the treatment with potent topical corticosteroids. The particularity of the case consists in the association of sclerotic lichen with 2 autoimmune diseases-diabetes mellitus and autoimmune thyroiditis, being rarer described in the literature this association and relatively late consultation due to the pandemic context.<\/p>\n

Lichenul scleros este o dermatoz\u0103 inflamatorie autoimun\u0103, cronic\u0103, scleroatrofic\u0103, ce afecteaz\u0103 regiunea anogenital\u0103 \u00een procent de peste 85% din cazuri \u0219i \u00eentr-o mic\u0103 m\u0103sur\u0103 zonele extragenitale (15%). Este o boal\u0103 relativ rar\u0103, mai frecvent \u00eent\u00e2lnit\u0103 la femei de c\u00e2t la b\u0103rba\u021bi F:B=5:1, put\u00e2nd afecta inclusiv copiii. Etiologia bolii r\u0103m\u00e2ne necunoscut\u0103, fiind incomplet elucidat\u0103. Sunt incrimina\u021bi factori genetici, autoimuni, infec\u021bio\u0219i, hormonali. Cea mai grav\u0103 complica\u021bie este reprezentat\u0103 de apari\u021bia carcinomului scuamocelular ce poate apare la aproximativ 10 % din b\u0103rba\u021bi. Sunt propuse mai multe alternative terapeutice pentru leziunile genitale, cea mai utilizat\u0103 fiind corticoterapia local\u0103. Prezent\u0103m cazul unui pacient HP, \u00een v\u00e2rst\u0103 de 42 ani, ce se adreseaz\u0103 pentru apari\u021bia de aproximativ 8 luni a unor pl\u0103ci u\u0219or indurate, albicioase, cu zone pigmentate, la nivel penian, asociate cu usturime local\u0103, durere la contact sexual, prurit. Pacientul asociaz\u0103 de 6 ani un diabet zaharat tip 2, cu evolu\u021bie ondulant\u0103 \u0219i de 5 ani tiroidit\u0103 autoimun\u0103. Examenul clinic a fost completat cu dermatoscopie \u0219i ex histopatologic care a transat diagnosticul de lichen scleros. Evolu\u021bia a fost favorabil\u0103 sub tratamentul cu corticosteroizi topici poten\u021bi.
\nParticularitatea cazului const\u0103 \u00een asocierea lichenului scleros cu 2 afec\u021biuni autoimune-diabet zaharat \u0219i tiroidit\u0103 autoimun\u0103, fiind mai rar\u0103 descris\u0103 \u00een literatura de specialitate aceast\u0103 asociere \u0219i adresarea relativ tardiv\u0103 la consult datorit\u0103 contextului pandemic.<\/p>\n
Penian sclerosis lichen associated with type 2 diabet and autoimmune thyroiditis<\/span> Keywords: lichen sclerosus, autoimmune diseases, diagnosis<\/p>\n Sclerotic lichen is an autoimmune, chronic, scleroatrophic inflammatory dermatosis that affects the anogenital region in over 85% of cases and to a lesser extent the extragenital areas (15%). It is a relatively rare disease, more common in women than in men; F:B = 5: 1, and can even affect children. The etiology of the disease remains unknown, being incompletely elucidated. Genetic, autoimmune, infectious, hormonal factors are incriminated. The most serious complication is the appearance of squamous cell carcinoma that can occur in about 10% of men. Several therapeutic alternatives are proposed for genital lesions, the most used being local corticotherapy. We present the case of a patient, HP, aged 42 years, who addresses for the presence of approximately 8 months slightly hardened, whitish plaques, with pigmented areas, at the penile level, associated with local stinging, pain in sexual contact, pruritus. The patient has been associated with type 2 diabetes for 6 years, with undulating evolution and autoimmune thyroiditis for 5 years. The clinical examination was supplemented with dermatoscopy and histopathological examination that diagnosed lichen sclerosis. The evolution was favorable under the treatment with potent topical corticosteroids.
