Peste 50% dintre pacien\u021bii cu boal\u0103 renal\u0103 \u00een diferite stadii evolutive: predializ\u0103 sau dializ\u0103, au cel pu\u021bin o manifestare cutanat\u0103 asociat\u0103. Deoarece pielea ac\u021bioneaz\u0103 frecvent ca o oglind\u0103 a bolilor sistemice subiacente, recunoa\u0219terea indiciilor clinice subtile cutanate ar putea ajuta la diagnosticarea precoce a disfunc\u021biei renale.
Material \u0219i metod\u0103. Modific\u0103rile cutanate \u00een boala cronic\u0103 de rinichi sever\u0103 cuprind:
1. Manifest\u0103ri cutanate asociate BCR: xeroza cutanat\u01ce, pruritul, modific\u01cerile pigmentare, dermatozele perforante dob\u00e2ndite, (frecvent\u01ce la diabetic, apare la cca 11% dintre dializa\u021bi), calcifilaxia \u0219i calcinoza cutanat\u0103, ambele asociate hiperparatiroidismului secundar, boli buloase (porfiria cutanat\u0103 tardiv\u0103 \u0219i pseudoporfiria), xantomul eruptiv, iododermia \u0219i fibroza sistemic\u0103 nefrogen\u0103. Fibroza sistemic\u0103 nefrogen\u0103, o afectare cutanat\u0103 scleroderma-like, cauzat\u0103 de expunerea la agen\u021bi de contrast pe baz\u0103 de gadoliniu la pacien\u021bii cu disfunc\u021bie renal\u0103, este considerat\u0103 a fi specific\u0103 pentru BCR stadiul terminal
2. Manifest\u0103ri cutanate frecvent asociate dializei: i) amiloidoza secundar\u0103 dializei, determinat\u0103 de depunerea de beta2-microglobulin\u0103, caracterizat\u0103 prin triada: sindrom de tunel carpian, periartrit\u0103 scapulohumeral\u0103 \u0219i tenosinovita flexoare a m\u00e2inii); ii) sindromul de furt vascular, o complica\u021bie rar\u0103 a c\u0103ii de acces vascular pentru hemodializ\u0103; iii) Pseudosarcomul Kaposi, o angiodisplazie a vasculariza\u021biei subcutanate, asociate \u0219untului arterio-venos al hemodializatului.
Concluzii. Manifest\u0103rile cutanate sunt destul de frecvente la pacien\u021bii cu BCR, iar uneori pot fi primul indiciu evident al bolii renale subiacente. Recunoa\u0219terea \u0219i gestionarea prompt\u0103 a manifest\u0103rilor dermatologice prin echipa nefrolog-dermatolog, va reduce considerabil morbiditatea \u0219i va \u00eembun\u0103t\u0103\u021bi calitatea vie\u021bii acestor pacien\u021bi. <\/p>\n

Introduction. Chronic kidney disease (CKD) affects about 10% of the world’s population. Over 50% of patients with CKD in different stages: predialysis or dialysis, have at least one associated cutaneous manifestation. Skin acts frequently like a mirror of underlying systemic diseases, so recognition of subtle cutaneous clinical clues could help in an early diagnosis of renal disorders.
Material and method. Skin involvement in end-stage renal disease (ESRD) include:
1. Cutaneous manifestations associated with ESRD: cutaneous xerosis, pruritus, pigmentary changes, acquired perforating dermatoses (common in diabetics, occurs in about 11% of dialysis patients), calciphylaxis and cutaneous calcinosis, both associated with secondary hyperparathyroidism, bullous diseases (porphyria cutanea tarda and pseudoporphyria), eruptive xanthoma, iododerma and nephrogenic systemic fibrosis. Nephrogenic systemic fibrosis, a scleroderma-like disorder caused by exposure to gadolinium-based contrast agents in patients with renal impairment, is considered to be specific for ESRD.
2. Skin manifestations frequently associated with dialysis: i) dialysis-related amyloidosis, due to deposition of beta2-microglobulin amyloid fibrils and characterized by the triad: carpal tunnel syndrome, scapulohumeral periarthritis and flexor tenosynovitis of the hand; ii) dialysis access steal syndrome, a rare complication of the vascular access for hemodialysis; iii) Pseudo Kaposi sarcoma, a reactive angiodysplasia of cutaneous vasculature, secondary to arterio-venous shunt.
Conclusions. Skin manifestations are quite common in patients with end-stage renal disease and can sometimes be the first obvious indication of underlying kidney disease. Prompt recognition and management of dermatological manifestations by the nephrologist-dermatologist team will improve the quality of life and life expectancy of these patients.<\/p>\n

Introducere. Boala cronic\u0103 de rinichi (BCR) afecteaz\u0103 aprox 10% din popula\u021bia lumii. Peste 50% dintre pacien\u021bii cu boal\u0103 renal\u0103 \u00een diferite stadii evolutive: predializ\u0103 sau dializ\u0103, au cel pu\u021bin o manifestare cutanat\u0103 asociat\u0103. Deoarece pielea ac\u021bioneaz\u0103 frecvent ca o oglind\u0103 a bolilor sistemice subiacente, recunoa\u0219terea indiciilor clinice subtile cutanate ar putea ajuta la diagnosticarea precoce a disfunc\u021biei renale.
Material \u0219i metod\u0103. Modific\u0103rile cutanate \u00een boala cronic\u0103 de rinichi sever\u0103 cuprind:
1. Manifest\u0103ri cutanate asociate BCR: xeroza cutanat\u01ce, pruritul, modific\u01cerile pigmentare, dermatozele perforante dob\u00e2ndite, (frecvent\u01ce la diabetic, apare la cca 11% dintre dializa\u021bi), calcifilaxia \u0219i calcinoza cutanat\u0103, ambele asociate hiperparatiroidismului secundar, boli buloase (porfiria cutanat\u0103 tardiv\u0103 \u0219i pseudoporfiria), xantomul eruptiv, iododermia \u0219i fibroza sistemic\u0103 nefrogen\u0103. Fibroza sistemic\u0103 nefrogen\u0103, o afectare cutanat\u0103 scleroderma-like, cauzat\u0103 de expunerea la agen\u021bi de contrast pe baz\u0103 de gadoliniu la pacien\u021bii cu disfunc\u021bie renal\u0103, este considerat\u0103 a fi specific\u0103 pentru BCR stadiul terminal
2. Manifest\u0103ri cutanate frecvent asociate dializei: i) amiloidoza secundar\u0103 dializei, determinat\u0103 de depunerea de beta2-microglobulin\u0103, caracterizat\u0103 prin triada: sindrom de tunel carpian, periartrit\u0103 scapulohumeral\u0103 \u0219i tenosinovita flexoare a m\u00e2inii); ii) sindromul de furt vascular, o complica\u021bie rar\u0103 a c\u0103ii de acces vascular pentru hemodializ\u0103; iii) Pseudosarcomul Kaposi, o angiodisplazie a vasculariza\u021biei subcutanate, asociate \u0219untului arterio-venos al hemodializatului.
Concluzii. Manifest\u0103rile cutanate sunt destul de frecvente la pacien\u021bii cu BCR, iar uneori pot fi primul indiciu evident al bolii renale subiacente. Recunoa\u0219terea \u0219i gestionarea prompt\u0103 a manifest\u0103rilor dermatologice prin echipa nefrolog-dermatolog, va reduce considerabil morbiditatea \u0219i va \u00eembun\u0103t\u0103\u021bi calitatea vie\u021bii acestor pacien\u021bi. <\/p>\n
Specific skin manifestations in patients with end-stage renal disease- predialysis and dialysis

Introduction. Chronic kidney disease (CKD) affects about 10% of the world’s population. Over 50% of patients with CKD in different stages: predialysis or dialysis, have at least one associated cutaneous manifestation. Skin acts frequently like a mirror of underlying systemic diseases, so recognition of subtle cutaneous clinical clues could help in an early diagnosis of renal disorders.
Material and method. Skin involvement in end-stage renal disease (ESRD) include:
1. Cutaneous manifestations associated with ESRD: cutaneous xerosis, pruritus, pigmentary changes, acquired perforating dermatoses (common in diabetics, occurs in about 11% of dialysis patients), calciphylaxis and cutaneous calcinosis, both associated with secondary hyperparathyroidism, bullous diseases (porphyria cutanea tarda and pseudoporphyria), eruptive xanthoma, iododerma and nephrogenic systemic fibrosis. Nephrogenic systemic fibrosis, a scleroderma-like disorder caused by exposure to gadolinium-based contrast agents in patients with renal impairment, is considered to be specific for ESRD.
2. Skin manifestations frequently associated with dialysis: i) dialysis-related amyloidosis, due to deposition of beta2-microglobulin amyloid fibrils and characterized by the triad: carpal tunnel syndrome, scapulohumeral periarthritis and flexor tenosynovitis of the hand; ii) dialysis access steal syndrome, a rare complication of the vascular access for hemodialysis; iii) Pseudo Kaposi sarcoma, a reactive angiodysplasia of cutaneous vasculature, secondary to arterio-venous shunt.
Conclusions. Skin manifestations are quite common in patients with end-stage renal disease and can sometimes be the first obvious indication of underlying kidney disease. Prompt recognition and management of dermatological manifestations by the nephrologist-dermatologist team will improve the quality of life and life expectancy of these patients.<\/p>\n

Material \u0219i metod\u0103. Modific\u0103rile cutanate \u00een boala cronic\u0103 de rinichi sever\u0103 cuprind:
1. Manifest\u0103ri cutanate asociate BCR: xeroza cutanat\u01ce, pruritul, modific\u01cerile pigmentare, dermatozele perforante dob\u00e2ndite, (frecvent\u01ce la diabetic, apare la cca 11% dintre dializa\u021bi), calcifilaxia \u0219i calcinoza cutanat\u0103, ambele asociate hiperparatiroidismului secundar, boli buloase (porfiria cutanat\u0103 tardiv\u0103 \u0219i pseudoporfiria), xantomul eruptiv, iododermia \u0219i fibroza sistemic\u0103 nefrogen\u0103. Fibroza sistemic\u0103 nefrogen\u0103, o afectare cutanat\u0103 scleroderma-like, cauzat\u0103 de expunerea la agen\u021bi de contrast pe baz\u0103 de gadoliniu la pacien\u021bii cu disfunc\u021bie renal\u0103, este considerat\u0103 a fi specific\u0103 pentru BCR stadiul terminal
2. Manifest\u0103ri cutanate frecvent asociate dializei: i) amiloidoza secundar\u0103 dializei, determinat\u0103 de depunerea de beta2-microglobulin\u0103, caracterizat\u0103 prin triada: sindrom de tunel carpian, periartrit\u0103 scapulohumeral\u0103 \u0219i tenosinovita flexoare a m\u00e2inii); ii) sindromul de furt vascular, o complica\u021bie rar\u0103 a c\u0103ii de acces vascular pentru hemodializ\u0103; iii) Pseudosarcomul Kaposi, o angiodisplazie a vasculariza\u021biei subcutanate, asociate \u0219untului arterio-venos al hemodializatului.
Concluzii. Manifest\u0103rile cutanate sunt destul de frecvente la pacien\u021bii cu BCR, iar uneori pot fi primul indiciu evident al bolii renale subiacente. Recunoa\u0219terea \u0219i gestionarea prompt\u0103 a manifest\u0103rilor dermatologice prin echipa nefrolog-dermatolog, va reduce considerabil morbiditatea \u0219i va \u00eembun\u0103t\u0103\u021bi calitatea vie\u021bii acestor pacien\u021bi. <\/p>\n

Material and method. Skin involvement in end-stage renal disease (ESRD) include:
1. Cutaneous manifestations associated with ESRD: cutaneous xerosis, pruritus, pigmentary changes, acquired perforating dermatoses (common in diabetics, occurs in about 11% of dialysis patients), calciphylaxis and cutaneous calcinosis, both associated with secondary hyperparathyroidism, bullous diseases (porphyria cutanea tarda and pseudoporphyria), eruptive xanthoma, iododerma and nephrogenic systemic fibrosis. Nephrogenic systemic fibrosis, a scleroderma-like disorder caused by exposure to gadolinium-based contrast agents in patients with renal impairment, is considered to be specific for ESRD.
2. Skin manifestations frequently associated with dialysis: i) dialysis-related amyloidosis, due to deposition of beta2-microglobulin amyloid fibrils and characterized by the triad: carpal tunnel syndrome, scapulohumeral periarthritis and flexor tenosynovitis of the hand; ii) dialysis access steal syndrome, a rare complication of the vascular access for hemodialysis; iii) Pseudo Kaposi sarcoma, a reactive angiodysplasia of cutaneous vasculature, secondary to arterio-venous shunt.
Conclusions. Skin manifestations are quite common in patients with end-stage renal disease and can sometimes be the first obvious indication of underlying kidney disease. Prompt recognition and management of dermatological manifestations by the nephrologist-dermatologist team will improve the quality of life and life expectancy of these patients.<\/p>\n
