Atopic dermatitis (AD), also known as atopic eczema, is a chronic or recurrent inflammatory skin condition that has a significant social and economic impact worldwide and affects 2.1% to 4.9% of the adult population, and 15-20% of the pediatric population. An increasing prevalence of AD has been reported, especially in low-income countries. Recent studies recognize a predilection of AD for persistence in adulthood, with a lifetime prevalence of 34.1%. AD is a debilitating skin condition in both children and adults, and severe, refractory forms of disaese are a challenge for both clinicians and patients alike.
Originally considered an allergic skin condition, AD is currently interpreted in the context of the complex interaction between genetic predisposition to epidermal barrier dysfunction and \/ or altered skin lipid composition and interaction with environmental factors. Understanding the pathological pathways that mediate inflammation in AD is the cornerstone of improved management of patients with this condition.
This paper reviews new immunomodulatory therapies in severe atopic dermatitis refractory to conventional treatment. Current data argue for the efficacy of Dupliumab and several inhibitors of the JAK signaling pathway, upadacitinib, abrocitinib, ruxolitinib, while other immunomodulatory agents such as nemolizumab, lebrikizumab, tralokinumab, ustekinumab are promising for their usefulness and safety in atopic dermatitis. Allergen-specific immunotherapy may be helpful in selected cases.<\/p>\n

Dermatita atopic\u0103 (DA), cunoscut\u0103 \u0219i sub denumirea de eczem\u0103 atopic\u0103, este o afec\u021biune inflamatorie a pielii cronic\u0103 sau recurent\u0103, ce aduce un impact social \u0219i economic semnificativ la nivel mondial \u0219i afecteaz\u0103 2,1% p\u00e2n\u0103 la 4,9% din popula\u021bia adult\u0103 \u0219i 15-20% dintre copii. Atopic dermatitis – molecular mechanisms and biological therapies<\/span> Keywords: atopic dermatitis, biological therapy, molecular mechanisms<\/p>\n Atopic dermatitis (AD), also known as atopic eczema, is a chronic or recurrent inflammatory skin condition that has a significant social and economic impact worldwide and affects 2.1% to 4.9% of the adult population, and 15-20% of the pediatric population. An increasing prevalence of AD has been reported, especially in low-income countries. Recent studies recognize a predilection of AD for persistence in adulthood, with a lifetime prevalence of 34.1%. Originally considered an allergic skin condition, AD is currently interpreted in the context of the complex interaction between genetic predisposition to epidermal barrier dysfunction and \/ or altered skin lipid composition and interaction with environmental factors. Understanding the pathological pathways that mediate inflammation in AD is the cornerstone of improved management of patients with this condition.
This paper reviews new immunomodulatory therapies in severe atopic dermatitis refractory to conventional treatment. Current data argue for the efficacy of Dupliumab and several inhibitors of the JAK signaling pathway, upadacitinib, abrocitinib, ruxolitinib, while other immunomodulatory agents such as nemolizumab, lebrikizumab, tralokinumab, ustekinumab are promising for their usefulness and safety in atopic dermatitis. Allergen-specific immunotherapy may be helpful in selected cases.<\/p>\n
