Totu\u0219i, sunt extrem de pu\u021bine date referitor la multitudinea de afec\u021biuni autoimmune care pot complica lupusul. Deasemenea, asocierea tiroiditei autoimune cu alte boli autoimune endocrine se reg\u0103se\u0219te \u00een cadrul poliendocrinopatiei multiple de tip III. Pacien\u021bi \u0219i metod\u0103. Prezentare de caz \u0219i punere la punct a literaturii. Rezultate. Prezent\u0103m cazul unei tinere de 20 de ani cu febr\u0103, cefalee, gre\u021buri, m\u0103rirea de volum a tiroidei \u0219i eritem polimorf la nivelul fe\u021bei \u0219i bra\u021belor. Ultrasonografia tiroidian\u0103, titrul anticorpilor anti-tiroidieni \u0219i valoarea TSH \u0219i fT4 relev\u0103 o tiroidit\u0103 autoimun\u0103 cu hipotiroidie sever\u0103. Examenul dermatologic coroborat cu titrurile crescute de anticorpi anti nucleari (ANA) \u0219i aDNAds stabile\u0219te diagnosticul de lupus eritematos. \u00cen aceste condi\u021bii tratamentul concomitent cu Levotiroxin\u0103 \u0219i Medrol a fost ini\u021biat. Rezultate. Tratamentul substitutiv cu hormoni tiroidieni a normalizat valorile TSH \u0219i fT4 dup\u0103 6 s\u0103pt\u0103m\u00e2ni iar corticoterapia a dus la ameliorarea leziunilor cutanate. Concluzii. Poliendocrinopatiile multiple reprezint\u0103 un grup heterogen de afec\u021biuni rare, definit prin co-existen\u021ba a cel pu\u021bin dou\u0103 afec\u021biuni autoimmune endocrine. \u00cen prezent sunt descries c\u00e2teva tipuri. Cele de tip III reprezint\u0103 co-existen\u021ba tiroiditei autoimmune cu o alt\u0103 afec\u021biune autoimun\u0103 fie ea endocrin\u0103 sau nu. Introduction. The association between systemic lupus erythematosus (SLE) and other
autoimmune diseases such as antiphospholipid syndrome, Sjogren\u2019s syndrome or autoimmune thyroiditis have been described. However, there is a paucity of studies looking comprehensively at the wider range of other autoimmune disorders that complicate lupus. Furthermore, autoimmune thyroiditis in association with other autoimmune diseases is known as type 3 polyendocrine autoimmune syndrome (PAS). Patient and Method. Case report with a review of the literature. Results. We present the case of a 20 years old female with concomitant fever, headache, nausea, swelling of thyroid gland and polymorphic erythema on arms and face. The thyroid ultrasound, the titer of anti-thyroid antibodies and level of TSH and fT4 revealed an autoimmune thyroiditis with severe hypothyroidism. The dermatological examination and the elevated titers of antinuclear (ANA) and anti-double stranded DNA antibodies (aDNAds) established the diagnosis of lupus erythematosus. The concomitant treatment with levothyroxine and medrol was started. Results. The substitutive treatment with thyroid hormones normalized the level of TSH and fT4 in six weeks also the corticotherapy improved the skin lesions. Conclusions. PAS represent a heterogeneous group of rare conditions, defined by the coexistence of at least two endocrine autoimmune disorders. Currently several types are described. PAS type III is defined as the coexistence of autoimmune thyroid disorder with other endocrine or non-endocrine autoimmune condition. Our case meets the criteria for PAS type III but we can\u2019t rule out a mere coincidence.<\/p>\n

Introducere. Asocierea dintre lupusul eritematos (LE) \u0219i alte boli autoimmune cum ar fi: sindromul anti-fosfolipidic, sindromul Sjogren sau tiroidita autoimun\u0103 a fost descris\u0103 \u00een literatur\u0103. Totu\u0219i, sunt extrem de pu\u021bine date referitor la multitudinea de afec\u021biuni autoimmune care pot complica lupusul. Deasemenea, asocierea tiroiditei autoimune cu alte boli autoimune endocrine se reg\u0103se\u0219te \u00een cadrul poliendocrinopatiei multiple de tip III. Pacien\u021bi \u0219i metod\u0103. Prezentare de caz \u0219i punere la punct a literaturii. Rezultate. Prezent\u0103m cazul unei tinere de 20 de ani cu febr\u0103, cefalee, gre\u021buri, m\u0103rirea de volum a tiroidei \u0219i eritem polimorf la nivelul fe\u021bei \u0219i bra\u021belor. Ultrasonografia tiroidian\u0103, titrul anticorpilor anti-tiroidieni \u0219i valoarea TSH \u0219i fT4 relev\u0103 o tiroidit\u0103 autoimun\u0103 cu hipotiroidie sever\u0103. Examenul dermatologic coroborat cu titrurile crescute de anticorpi anti nucleari (ANA) \u0219i aDNAds stabile\u0219te diagnosticul de lupus eritematos. \u00cen aceste condi\u021bii tratamentul concomitent cu Levotiroxin\u0103 \u0219i Medrol a fost ini\u021biat. Rezultate. Tratamentul substitutiv cu hormoni tiroidieni a normalizat valorile TSH \u0219i fT4 dup\u0103 6 s\u0103pt\u0103m\u00e2ni iar corticoterapia a dus la ameliorarea leziunilor cutanate. Concluzii. Poliendocrinopatiile multiple reprezint\u0103 un grup heterogen de afec\u021biuni rare, definit prin co-existen\u021ba a cel pu\u021bin dou\u0103 afec\u021biuni autoimmune endocrine. \u00cen prezent sunt descries c\u00e2teva tipuri. Cele de tip III reprezint\u0103 co-existen\u021ba tiroiditei autoimmune cu o alt\u0103 afec\u021biune autoimun\u0103 fie ea endocrin\u0103 sau nu. Cazul nostru \u00eentrune\u0219te condi\u021biile de ploiendocrinopatie autoimun\u0103 de tip III dar nu putem exclude cu certitudine \u0219i o simpl\u0103 concomiten\u021b\u0103.<\/p>\n

Introduction. The association between systemic lupus erythematosus (SLE) and other
autoimmune diseases such as antiphospholipid syndrome, Sjogren\u2019s syndrome or autoimmune thyroiditis have been described. However, there is a paucity of studies looking comprehensively at the wider range of other autoimmune disorders that complicate lupus. Furthermore, autoimmune thyroiditis in association with other autoimmune diseases is known as type 3 polyendocrine autoimmune syndrome (PAS). Patient and Method. Case report with a review of the literature. Results. We present the case of a 20 years old female with concomitant fever, headache, nausea, swelling of thyroid gland and polymorphic erythema on arms and face. The thyroid ultrasound, the titer of anti-thyroid antibodies and level of TSH and fT4 revealed an autoimmune thyroiditis with severe hypothyroidism. The dermatological examination and the elevated titers of antinuclear (ANA) and anti-double stranded DNA antibodies (aDNAds) established the diagnosis of lupus erythematosus. The concomitant treatment with levothyroxine and medrol was started. Results. The substitutive treatment with thyroid hormones normalized the level of TSH and fT4 in six weeks also the corticotherapy improved the skin lesions. Conclusions. PAS represent a heterogeneous group of rare conditions, defined by the coexistence of at least two endocrine autoimmune disorders. Currently several types are described. PAS type III is defined as the coexistence of autoimmune thyroid disorder with other endocrine or non-endocrine autoimmune condition. Our case meets the criteria for PAS type III but we can\u2019t rule out a mere coincidence.<\/p>\n
Scurt CV<\/span><\/p>\nDr. Cristina Preda is currently Professor of Endocrinology, Head of the Department of Endocrinology of the University of Medicine and Pharmacy \u201dGrigore T. Popa\u201d and Head of the Endocrinology Clinic of the University Hospital \u201cSt. Spiridon\u201d, Iasi, Romania. She received her medical degree and her PhD from \u201cGrigore T. Popa\u201d University of Medicine and Pharmacy. Her research interest covers area as: PCOS, thyroid pathology, GH disorders. Author and co-author of 22 books and 60 articles in national and international peer-reviewed journals. She is member of the Board of Romanian Society of Endocrinology since 2014 and Vice-President since 2017.<\/p>\n

Prezent\u0103m cazul unei tinere de 20 de ani cu febr\u0103, cefalee, gre\u021buri, m\u0103rirea de volum a tiroidei \u0219i eritem polimorf la nivelul fe\u021bei \u0219i bra\u021belor. Ultrasonografia tiroidian\u0103, titrul anticorpilor anti-tiroidieni \u0219i valoarea TSH \u0219i fT4 relev\u0103 o tiroidit\u0103 autoimun\u0103 cu hipotiroidie sever\u0103. Examenul dermatologic coroborat cu titrurile crescute de anticorpi anti nucleari (ANA) \u0219i aDNAds stabile\u0219te diagnosticul de lupus eritematos. \u00cen aceste condi\u021bii tratamentul concomitent cu Levotiroxin\u0103 \u0219i Medrol a fost ini\u021biat. Rezultate. Tratamentul substitutiv cu hormoni tiroidieni a normalizat valorile TSH \u0219i fT4 dup\u0103 6 s\u0103pt\u0103m\u00e2ni iar corticoterapia a dus la ameliorarea leziunilor cutanate. Concluzii. Poliendocrinopatiile multiple reprezint\u0103 un grup heterogen de afec\u021biuni rare, definit prin co-existen\u021ba a cel pu\u021bin dou\u0103 afec\u021biuni autoimmune endocrine. \u00cen prezent sunt descries c\u00e2teva tipuri. Cele de tip III reprezint\u0103 co-existen\u021ba tiroiditei autoimmune cu o alt\u0103 afec\u021biune autoimun\u0103 fie ea endocrin\u0103 sau nu. autoimmune diseases such as antiphospholipid syndrome, Sjogren\u2019s syndrome or autoimmune thyroiditis have been described. However, there is a paucity of studies looking comprehensively at the wider range of other autoimmune disorders that complicate lupus. Furthermore, autoimmune thyroiditis in association with other autoimmune diseases is known as type 3 polyendocrine autoimmune syndrome (PAS). Patient and Method. Case report with a review of the literature. Results. We present the case of a 20 years old female with concomitant fever, headache, nausea, swelling of thyroid gland and polymorphic erythema on arms and face. The thyroid ultrasound, the titer of anti-thyroid antibodies and level of TSH and fT4 revealed an autoimmune thyroiditis with severe hypothyroidism. The dermatological examination and the elevated titers of antinuclear (ANA) and anti-double stranded DNA antibodies (aDNAds) established the diagnosis of lupus erythematosus. The concomitant treatment with levothyroxine and medrol was started. Results. The substitutive treatment with thyroid hormones normalized the level of TSH and fT4 in six weeks also the corticotherapy improved the skin lesions. Conclusions. PAS represent a heterogeneous group of rare conditions, defined by the coexistence of at least two endocrine autoimmune disorders. Currently several types are described. PAS type III is defined as the coexistence of autoimmune thyroid disorder with other endocrine or non-endocrine autoimmune condition. Our case meets the criteria for PAS type III but we can\u2019t rule out a mere coincidence.<\/p>\n<\/span><\/p>\n
