Pacientul SI, de 68 ani a fost internat \u00een repetate r\u00e2nduri \u00een Clinica Dermatologic\u0103 Timi\u0219oara pentru un prurigo rebel, debutat \u00een urm\u0103 cu 7 ani. Investiga\u021biile paraclinice uzuale \u0219i consulturile efectuate s-au soldat cu date normale, cu excep\u021bia unei hemoragii oculte \u00een scaun, la ultima internare, pentru care colonoscopia a relevat prezenta unui polip. Tratamentele cu complexitate cresc\u00e2nd\u0103 (antihistaminice H1, H2, antileucotriene, corticosteroizi) au r\u0103mas f\u0103r\u0103 efect, pruritul devenind at\u00e2t de devastator inc\u00e2t a dus la o tentativ\u0103 de suicid. Am extins investiga\u021biile cu efectuarea anticorpilor anti virus Epstein-Barr (EBV) \u0219i citomegalic (CMV) cu ob\u021binerea unor rezultate pozitive, mai ales la EBV. Am instituit tratament cu Ivermectin\u0103 0,02 mg\/kg, urmat, spre surprinderea noastr\u0103, de remiterea dramatic\u0103, aproape total\u0103, a simptomelor, \u00een decursul aceleia\u0219i nop\u021bi.
In concluzie, investigarea EBV \u0219i CMV trebuie avut\u0103 \u00een vedere \u00een prurigourile cronice recalcitrante, iar, \u00een caz de pozitivitate, Ivermectina poate fi o op\u021biune terapeutic\u0103 eficient\u0103.<\/p>\n
Etiological surprise and unusual treatament in a case of rebellious prurigo<\/span>

Pacient SI, aged 68 was repeatedly admitted in Timi\u0219oara Dermatological Clinic for a rebellious prurigo, which started 7 years ago. The complex investigations we did revealed normal data, except for an occult stool hemorrhage (the colonoscopy revealed a colonic polyp) .Treatament of increasing complexity (antihistamins H1, H2, antileucotriens, corticosteroids) remained ineffective and pruritus became so devastating that it lead to a suicidal attempt. We extended the tests and performed Epstein-Barr (EBV) and cytomegalic (CMV) virus antibodies with positive results, especially for EBV. We started Ivermectin\u0103 0,02 mg\/kg, which led, to our surprise, to a dramatic, almost complete, improvement of the symptoms during the very following night.
In conclusion, investigation of EBV and CMV should be considered in chronic reluctant prurigos and, if positive, Ivermectin may be an efficient terapeutic option.<\/p>\n

\u201cDog ears\u201d are mamilated elevations which form in the extremities of fusiform excisions when their length\/width ratio is less than 2,5:1 or at the origin of rotation flaps. As a rule, this estetic defect is fixed by a suplimentary small triangular excision, angulated to the initial incision \u2013 the so-called \u201cBurow triangles\u201d. The first technique we propose uses a towel clip in order to set the excision line of the Burow triangle in the same axis as the initial excision line, with the benefit of a more estetic final scar (linear instead of angulated). The second technique aims at flattening the dog ears by a personal suture technique which will be thoroughly demonstrated by the photographic documentation.<\/p>\n

Popularizarea sondelor liniare de \u00eenalt\u0103 frecven\u021b\u0103 accesibile ca pre\u021b (peste 15 Mhz) a dus la un interes re\u00eennoit \u00een domeniul ecografiei dermatologice. Ecografia dermatologic\u0103 de \u00eenalt\u0103 frecven\u021b\u0103, prescurtat\u0103 HFUS \u00een literatura interna\u021bional\u0103 (high frequency skin ultrasound) ne d\u0103 acces la o lume nev\u0103zut\u0103 \u00een cadrul consulta\u021biei dermatologice, o lume \u00een care putem explora at\u00e2t parametrii morfologici ai profunzimii pielii, dar \u0219i parametrii func\u021bionali cutana\u021bi (circula\u021bie, elasticitate). Pe parcursul cursului postuniversitar de ecografie dermatologic\u0103 din cadrul Prim\u0103verii Dermatologice Ie\u0219ene v\u0103 ve\u021bi familiariza cu no\u021biunile de baz\u0103 ale morfologiei cutanate ecografice \u0219i patologiile principale care pot fi evaluate cu ajutorul acestor tehnici (patologie tumoral\u0103, ganglioni limfatici, obiecte str\u0103ine implantate \u00een piele, calcific\u0103ri ectopice). Ultrasound is a dynamic and promising area in many clinical specialties. The technical and financial obstacles for the widespread use of ultrasound are being overcome, and ultrasound is progressing towards becoming the\u201d stethoscope of the future\u201d. In Dermatology, linear high frequency probe ultrasound allows us to evaluate the skin layers both morphologically and functionally This provides us with an “in vivo\u201d biopsy of the tissue and allows us to evaluate blood flow and tissue elasticity. During our skin ultrasound course, the participants will be familiarized with the ultrasonographic aspects of normal and pathological skin. The main groups of diseases in which ultrasound can be useful will be discussed (tumors, evaluation of lymph nodes, foreign objects in the skin, ectopic skin calcifications). We will also review notable publications and present recent state-of-the-art advancements made by Romanian researchers in this field. Participants will also have the opportunity for small hands-on group sessions with ultrasound equipment.
We hope to demonstrate the utility of ultrasound, as well as its ease of adoption within a clinical setting for Dermatologists. We also wish to raise awareness of this technique and to lay the foundation for a future in which dermatologists confidently use ultrasound in their daily practice.<\/p>\n

Pacientul SI, de 68 ani a fost internat \u00een repetate r\u00e2nduri \u00een Clinica Dermatologic\u0103 Timi\u0219oara pentru un prurigo rebel, debutat \u00een urm\u0103 cu 7 ani. Investiga\u021biile paraclinice uzuale \u0219i consulturile efectuate s-au soldat cu date normale, cu excep\u021bia unei hemoragii oculte \u00een scaun, la ultima internare, pentru care colonoscopia a relevat prezenta unui polip. Tratamentele cu complexitate cresc\u00e2nd\u0103 (antihistaminice H1, H2, antileucotriene, corticosteroizi) au r\u0103mas f\u0103r\u0103 efect, pruritul devenind at\u00e2t de devastator inc\u00e2t a dus la o tentativ\u0103 de suicid. Am extins investiga\u021biile cu efectuarea anticorpilor anti virus Epstein-Barr (EBV) \u0219i citomegalic (CMV) cu ob\u021binerea unor rezultate pozitive, mai ales la EBV. Am instituit tratament cu Ivermectin\u0103 0,02 mg\/kg, urmat, spre surprinderea noastr\u0103, de remiterea dramatic\u0103, aproape total\u0103, a simptomelor, \u00een decursul aceleia\u0219i nop\u021bi.
In concluzie, investigarea EBV \u0219i CMV trebuie avut\u0103 \u00een vedere \u00een prurigourile cronice recalcitrante, iar, \u00een caz de pozitivitate, Ivermectina poate fi o op\u021biune terapeutic\u0103 eficient\u0103.<\/p>\n
Etiological surprise and unusual treatament in a case of rebellious prurigo<\/span>

Pacient SI, aged 68 was repeatedly admitted in Timi\u0219oara Dermatological Clinic for a rebellious prurigo, which started 7 years ago. The complex investigations we did revealed normal data, except for an occult stool hemorrhage (the colonoscopy revealed a colonic polyp) .Treatament of increasing complexity (antihistamins H1, H2, antileucotriens, corticosteroids) remained ineffective and pruritus became so devastating that it lead to a suicidal attempt. We extended the tests and performed Epstein-Barr (EBV) and cytomegalic (CMV) virus antibodies with positive results, especially for EBV. We started Ivermectin\u0103 0,02 mg\/kg, which led, to our surprise, to a dramatic, almost complete, improvement of the symptoms during the very following night.
In conclusion, investigation of EBV and CMV should be considered in chronic reluctant prurigos and, if positive, Ivermectin may be an efficient terapeutic option.<\/p>\n

\u201cDog ears\u201d are mamilated elevations which form in the extremities of fusiform excisions when their length\/width ratio is less than 2,5:1 or at the origin of rotation flaps. As a rule, this estetic defect is fixed by a suplimentary small triangular excision, angulated to the initial incision \u2013 the so-called \u201cBurow triangles\u201d. The first technique we propose uses a towel clip in order to set the excision line of the Burow triangle in the same axis as the initial excision line, with the benefit of a more estetic final scar (linear instead of angulated). The second technique aims at flattening the dog ears by a personal suture technique which will be thoroughly demonstrated by the photographic documentation.<\/p>\n

Ultrasound is a dynamic and promising area in many clinical specialties. The technical and financial obstacles for the widespread use of ultrasound are being overcome, and ultrasound is progressing towards becoming the\u201d stethoscope of the future\u201d. In Dermatology, linear high frequency probe ultrasound allows us to evaluate the skin layers both morphologically and functionally This provides us with an “in vivo\u201d biopsy of the tissue and allows us to evaluate blood flow and tissue elasticity. During our skin ultrasound course, the participants will be familiarized with the ultrasonographic aspects of normal and pathological skin. The main groups of diseases in which ultrasound can be useful will be discussed (tumors, evaluation of lymph nodes, foreign objects in the skin, ectopic skin calcifications). We will also review notable publications and present recent state-of-the-art advancements made by Romanian researchers in this field. Participants will also have the opportunity for small hands-on group sessions with ultrasound equipment.
We hope to demonstrate the utility of ultrasound, as well as its ease of adoption within a clinical setting for Dermatologists. We also wish to raise awareness of this technique and to lay the foundation for a future in which dermatologists confidently use ultrasound in their daily practice.<\/p>\n

As a rule, this estetic defect is fixed by a suplimentary small triangular excision, angulated to the initial incision \u2013 the so-called \u201cBurow triangles\u201d. The first technique we propose uses a towel clip in order to set the excision line of the Burow triangle in the same axis as the initial excision line, with the benefit of a more estetic final scar (linear instead of angulated). Pacientul SI, de 68 ani a fost internat \u00een repetate r\u00e2nduri \u00een Clinica Dermatologic\u0103 Timi\u0219oara pentru un prurigo rebel, debutat \u00een urm\u0103 cu 7 ani. Investiga\u021biile paraclinice uzuale \u0219i consulturile efectuate s-au soldat cu date normale, cu excep\u021bia unei hemoragii oculte \u00een scaun, la ultima internare, pentru care colonoscopia a relevat prezenta unui polip. Tratamentele cu complexitate cresc\u00e2nd\u0103 (antihistaminice H1, H2, antileucotriene, corticosteroizi) au r\u0103mas f\u0103r\u0103 efect, pruritul devenind at\u00e2t de devastator inc\u00e2t a dus la o tentativ\u0103 de suicid. Am extins investiga\u021biile cu efectuarea anticorpilor anti virus Epstein-Barr (EBV) \u0219i citomegalic (CMV) cu ob\u021binerea unor rezultate pozitive, mai ales la EBV. Am instituit tratament cu Ivermectin\u0103 0,02 mg\/kg, urmat, spre surprinderea noastr\u0103, de remiterea dramatic\u0103, aproape total\u0103, a simptomelor, \u00een decursul aceleia\u0219i nop\u021bi.
In concluzie, investigarea EBV \u0219i CMV trebuie avut\u0103 \u00een vedere \u00een prurigourile cronice recalcitrante, iar, \u00een caz de pozitivitate, Ivermectina poate fi o op\u021biune terapeutic\u0103 eficient\u0103.<\/p>\n

Pacient SI, aged 68 was repeatedly admitted in Timi\u0219oara Dermatological Clinic for a rebellious prurigo, which started 7 years ago. The complex investigations we did revealed normal data, except for an occult stool hemorrhage (the colonoscopy revealed a colonic polyp) .Treatament of increasing complexity (antihistamins H1, H2, antileucotriens, corticosteroids) remained ineffective and pruritus became so devastating that it lead to a suicidal attempt. We extended the tests and performed Epstein-Barr (EBV) and cytomegalic (CMV) virus antibodies with positive results, especially for EBV. We started Ivermectin\u0103 0,02 mg\/kg, which led, to our surprise, to a dramatic, almost complete, improvement of the symptoms during the very following night.
In conclusion, investigation of EBV and CMV should be considered in chronic reluctant prurigos and, if positive, Ivermectin may be an efficient terapeutic option.<\/p>\n
