Costan<\/span><\/p>\nIntroducere: Carcinoamele scuamocelulare cutanate pot conduce la dezvoltarea adenopatiilor metastatice intraparotidiene sau cervicale, al c\u0103ror diagnostic precoce poate fi dificil \u00een absen\u021ba unei suspiciuni clinice ridicate. \u00cen cadrul acestui studiu s-a urm\u0103rit evaluarea prezen\u021bei \u0219i a caracteristicilor acestor leziuni secundare ganglionare.
Material \u0219i metod\u0103: \u00cen studiu au fost incluse cazuri de metastaze ganglionare cervicale \u0219i intraparotidiene de la carcinoame scuamocelulare cutanate, not\u00e2ndu-se datele privind localizarea tumorii primare, intervalul de timp p\u00e2n\u0103 la apari\u021bia metastazelor ganglionare, rezultatul explor\u0103rilor imagistice, tipul de tratament efectuat \u0219i evolu\u021bia postoperatorie.
Rezultate: Adenopatiile metastatice cervicale au fost prezente \u00een majoritatea cazurilor, necesit\u00e2nd realizarea evid\u0103rii ganglionare cervicale. Pentru tratamentul adenopatiilor intraparotidiene s-a practicat parotidectomia total\u0103 cu conservarea nervului facial, atunci c\u00e2nd a fost posibil, \u00eempreun\u0103 cu evidarea ganglionar\u0103 cervical\u0103 de aceea\u0219i parte. \u00cen multe cazuri, prezentarea a fost tardiv\u0103, necesit\u00e2nd practicarea unei parotidectomii extinse la tegument, pavilionul auricular \u0219i osul temporal, c\u00e2t \u0219i plastia defectului utiliz\u00e2nd lambouri liber transferate. Vertijul tranzitor postoperator a fost frecvent \u00eent\u00e2lnit \u00een aceste cazuri. De asemenea, au existat adenopatii metastatice extinse la tegument \u0219i la osul mandibular, rezult\u00e2nd defecte \u00eentinse ce au necesitat reconstruc\u021bia cu ajutorul unor lambouri compuse liber transferate pentru a ob\u021bine cele mai bune rezultate func\u021bionale \u0219i estetice.
Concluzii: Carcinomul scuamocelular cutanat este mai mult dec\u00e2t o patologie malign\u0103 local\u0103, necesit\u00e2nd o evaluare atent\u0103 a ganglionilor regionali \u0219i o urm\u0103rire adecvat\u0103 pe termen lung, viz\u00e2nd un diagnostic precoce al metastazelor ganglionare. Tratamentul adecvat \u00een raport cu particularit\u0103\u021bile localiz\u0103rii \u0219i cu extensia invaziei, are rezultate bune pe termen lung.<\/p>\n
Cutaneous squamous-cell carcinoma of the face – more than a local pathology!<\/span><\/p>\nV.V. Costan<\/span><\/p>\nIntroduction: Cutaneous squamous-cell carcinomas can lead to the development of intraparotid or cervical metastatic adenopathies, whose early diagnosis can be difficult in the absence of high clinical suspicion. This study aimed to evaluate the presence and characteristics of these secondary nodal lesions.
Material and method: Cases of cervical and intraparotid lymph node metastases from cutaneous squamous-cell carcinomas were included in the study, noting the data on the location of the primary tumor, the time interval until the appearance of lymph node metastases, the result of imaging studies, the type of treatment performed and the postoperative evolution.
Results: Cervical metastatic adenopathies were present in most cases, requiring cervical lymph node dissection. For the treatment of intraparotid adenopathies, total parotidectomy was performed with preservation of the facial nerve, when possible, along with cervical lymph node dissection on the same side. In many cases, the presentation was late, requiring a parotidectomy extended to the skin, external ear, and temporal bone, as well as the repair of the defect using free flaps. Transient postoperative vertigo was frequently encountered in these cases. There were also adenopathies extended to the skin and mandibular bone, resulting in extensive defects that required reconstruction with composite free flaps, to achieve the best functional and aesthetic outcomes.
Conclusions: Cutaneous squamous-cell carcinoma is more than a local malignancy, requiring careful evaluation of the regional nodes and adequate long-term follow-up, aiming at early diagnosis of the nodal metastases. Adequate treatment in relation to the particularities of the location and extent of invasion has good long-term results.<\/p>\n

Material \u0219i metod\u0103: \u00cen studiu au fost incluse cazuri de metastaze ganglionare cervicale \u0219i intraparotidiene de la carcinoame scuamocelulare cutanate, not\u00e2ndu-se datele privind localizarea tumorii primare, intervalul de timp p\u00e2n\u0103 la apari\u021bia metastazelor ganglionare, rezultatul explor\u0103rilor imagistice, tipul de tratament efectuat \u0219i evolu\u021bia postoperatorie.
Rezultate: Adenopatiile metastatice cervicale au fost prezente \u00een majoritatea cazurilor, necesit\u00e2nd realizarea evid\u0103rii ganglionare cervicale. Pentru tratamentul adenopatiilor intraparotidiene s-a practicat parotidectomia total\u0103 cu conservarea nervului facial, atunci c\u00e2nd a fost posibil, \u00eempreun\u0103 cu evidarea ganglionar\u0103 cervical\u0103 de aceea\u0219i parte. \u00cen multe cazuri, prezentarea a fost tardiv\u0103, necesit\u00e2nd practicarea unei parotidectomii extinse la tegument, pavilionul auricular \u0219i osul temporal, c\u00e2t \u0219i plastia defectului utiliz\u00e2nd lambouri liber transferate. Vertijul tranzitor postoperator a fost frecvent \u00eent\u00e2lnit \u00een aceste cazuri. De asemenea, au existat adenopatii metastatice extinse la tegument \u0219i la osul mandibular, rezult\u00e2nd defecte \u00eentinse ce au necesitat reconstruc\u021bia cu ajutorul unor lambouri compuse liber transferate pentru a ob\u021bine cele mai bune rezultate func\u021bionale \u0219i estetice.
Concluzii: Carcinomul scuamocelular cutanat este mai mult dec\u00e2t o patologie malign\u0103 local\u0103, necesit\u00e2nd o evaluare atent\u0103 a ganglionilor regionali \u0219i o urm\u0103rire adecvat\u0103 pe termen lung, viz\u00e2nd un diagnostic precoce al metastazelor ganglionare. Tratamentul adecvat \u00een raport cu particularit\u0103\u021bile localiz\u0103rii \u0219i cu extensia invaziei, are rezultate bune pe termen lung.<\/p>\n
Material and method: Cases of cervical and intraparotid lymph node metastases from cutaneous squamous-cell carcinomas were included in the study, noting the data on the location of the primary tumor, the time interval until the appearance of lymph node metastases, the result of imaging studies, the type of treatment performed and the postoperative evolution.
Results: Cervical metastatic adenopathies were present in most cases, requiring cervical lymph node dissection. For the treatment of intraparotid adenopathies, total parotidectomy was performed with preservation of the facial nerve, when possible, along with cervical lymph node dissection on the same side. In many cases, the presentation was late, requiring a parotidectomy extended to the skin, external ear, and temporal bone, as well as the repair of the defect using free flaps. Transient postoperative vertigo was frequently encountered in these cases. There were also adenopathies extended to the skin and mandibular bone, resulting in extensive defects that required reconstruction with composite free flaps, to achieve the best functional and aesthetic outcomes.
Conclusions: Cutaneous squamous-cell carcinoma is more than a local malignancy, requiring careful evaluation of the regional nodes and adequate long-term follow-up, aiming at early diagnosis of the nodal metastases. Adequate treatment in relation to the particularities of the location and extent of invasion has good long-term results.<\/p>\n
