Introducere: Pl\u0103gile cronice reprezint\u0103 un segment important din patologia \u00eent\u00e2lnit\u0103 \u00een clinicile chirurgicale \u0219i medicale iar vindecarea lor este, deseori, incomplet\u0103, necesit\u0103 spitalizare \u00eendelungat\u0103, numeroase interven\u021bii chirurgicale \u0219i consum mare de resurse.Procesul de vindecare al unei pl\u0103gi cronice depinde de factori generali (comorbidit\u0103\u021bi sau dezechilibre metabolice, etc) dar \u0219i de caracteristici loco-regionale.
Material \u0219i metod\u0103: Am analizat un num\u0103r de 64 de cazuri (31 femei) cu pl\u0103gi cronice de diverse etiologii (32 arsuri vechi, neglijate terapeutic 5-7 zile postcombustional; 14 pl\u0103gi chirurgicale complicate, cu evolu\u021bie trenant\u0103; 18 leziuni trofice- ulcere venoase\/arteriale, escare de decubit, ulcera\u021bii Marjolin postiradiere, ulcera\u021bii pe picior diabetic) tratate \u00eentr-o perioad\u0103 de timp de 7 ani. Pentru tratamentul acestor pl\u0103gi am aplicat protocoale terapeutice generale \u0219i locale.
Tratamentul local a urmat protocoale diferite care s\u0103 asigure detersia pl\u0103gii (42 cazuri), \u00eendep\u0103rtarea biofilmului (26 cazuri), stimularea dezvolt\u0103rii re\u021belei vasculare \u0219i \u021besutului granular (42 cazuri), protec\u021bia antibacterian\u0103 (54 cazuri), precum \u0219i epitelizarea spontan\u0103 (46 cazuri).
\u00cen toate cazurile analizate am monitorizat gradul de colonizare bacterian\u0103 (prin examene bacteriologice seriate ale secre\u021biilor din pl\u0103gi), dar \u0219i progresia procesului de dezvoltare al \u021besutului granular \u0219i epiteliului.
Rezultate: Rata zilnic\u0103 a procesului de vindecare a oscilat \u00eentre 3.22% din suprafa\u021ba ini\u021bial\u0103 a pl\u0103gii (\u00een cazul arsurilor) \u0219i 2.03% din suprafa\u021ba ini\u021bial\u0103 a pl\u0103gii (\u00een cazul pl\u0103gilor cronice de alt\u0103 etiologie).
Concluzii: Am constatat c\u0103 exist\u0103 corela\u021bii importante \u00eentre viteza de progresie a vindec\u0103rii \u0219i terapia local\u0103 aplicat\u0103: debridare enzimatic\u0103 lent\u0103 sau prin detersie umed\u0103, terapie cu presiune negativ\u0103 sau debridare excizional\u0103, topice antibacteriene cu sau f\u0103r\u0103 con\u021binut \u00een acid hialuronic. Introduction: Chronic wounds represent an important segment of the pathology treated in surgical and medical clinics. The chronic wounds healing process is often incomplete and takes long time hospitalization, needs several surgical sessions and may be very expensive.
Matherial and method: We analyzed a number of 64 cases (31 women) with chronic wounds of various etiologies (32 old burns, therapeutically neglected 5-7 days post-combustion ; 14 complicated surgical wounds with long evolution; 18 trophic lesions- venous\/arterial ulcers, decubitus bedsores, post-irradiation Marjolin ulceration, diabetic foot ulcers) treated in a period of time of 7 years. For the treatment of these wounds we have applied general and local therapeutic protocols. Local treatment followed different protocols to ensure wound detersion (42 cases), biofilm removal (26 cases), local vascular network faster development and granular tissue growth (42 cases), antibacterial protection (54 cases), as well as spontaneous epithelialization (46 cases).
In all the cases, we monitored the degree of bacterial colonization (through serial bacteriological examinations of wound secretions), the progression of the granular tissue and epithelium development.
Results: The daily rate of the healing process oscillated between values of 3.22% of the initial wound surface (in the case of burns) and 2.03% of the initial wound surface (in the case of chronic wounds of another etiology).
Conclusions: We discovered important correlations between the rate of healing process progression and the applied local therapy: delayed enzymatic debridement or wet detersion, negative pressure wound therapy or excisional debridement, antibacterial topical with or without hyaluronic acid content.
The use of hyaluronic acid topical and individualized intelligent dressings can effectively influence the wound healing, can reduce the number of days of medical care, but also of surgical sessions, as well as the need for general therapy with antibiotics.<\/p>\n

Introduction: Chronic wounds represent an important segment of the pathology treated in surgical and medical clinics. The chronic wounds healing process is often incomplete and takes long time hospitalization, needs several surgical sessions and may be very expensive.
Matherial and method: We analyzed a number of 64 cases (31 women) with chronic wounds of various etiologies (32 old burns, therapeutically neglected 5-7 days post-combustion ; 14 complicated surgical wounds with long evolution; 18 trophic lesions- venous\/arterial ulcers, decubitus bedsores, post-irradiation Marjolin ulceration, diabetic foot ulcers) treated in a period of time of 7 years. For the treatment of these wounds we have applied general and local therapeutic protocols. Local treatment followed different protocols to ensure wound detersion (42 cases), biofilm removal (26 cases), local vascular network faster development and granular tissue growth (42 cases), antibacterial protection (54 cases), as well as spontaneous epithelialization (46 cases).
In all the cases, we monitored the degree of bacterial colonization (through serial bacteriological examinations of wound secretions), the progression of the granular tissue and epithelium development.
Results: The daily rate of the healing process oscillated between values of 3.22% of the initial wound surface (in the case of burns) and 2.03% of the initial wound surface (in the case of chronic wounds of another etiology).
Conclusions: We discovered important correlations between the rate of healing process progression and the applied local therapy: delayed enzymatic debridement or wet detersion, negative pressure wound therapy or excisional debridement, antibacterial topical with or without hyaluronic acid content.
The use of hyaluronic acid topical and individualized intelligent dressings can effectively influence the wound healing, can reduce the number of days of medical care, but also of surgical sessions, as well as the need for general therapy with antibiotics.<\/p>\n

Introducere: Pl\u0103gile cronice reprezint\u0103 un segment important din patologia \u00eent\u00e2lnit\u0103 \u00een clinicile chirurgicale \u0219i medicale iar vindecarea lor este, deseori, incomplet\u0103, necesit\u0103 spitalizare \u00eendelungat\u0103, numeroase interven\u021bii chirurgicale \u0219i consum mare de resurse.Procesul de vindecare al unei pl\u0103gi cronice depinde de factori generali (comorbidit\u0103\u021bi sau dezechilibre metabolice, etc) dar \u0219i de caracteristici loco-regionale.
Material \u0219i metod\u0103: Am analizat un num\u0103r de 64 de cazuri (31 femei) cu pl\u0103gi cronice de diverse etiologii (32 arsuri vechi, neglijate terapeutic 5-7 zile postcombustional; 14 pl\u0103gi chirurgicale complicate, cu evolu\u021bie trenant\u0103; 18 leziuni trofice- ulcere venoase\/arteriale, escare de decubit, ulcera\u021bii Marjolin postiradiere, ulcera\u021bii pe picior diabetic) tratate \u00eentr-o perioad\u0103 de timp de 7 ani. Pentru tratamentul acestor pl\u0103gi am aplicat protocoale terapeutice generale \u0219i locale.
Tratamentul local a urmat protocoale diferite care s\u0103 asigure detersia pl\u0103gii (42 cazuri), \u00eendep\u0103rtarea biofilmului (26 cazuri), stimularea dezvolt\u0103rii re\u021belei vasculare \u0219i \u021besutului granular (42 cazuri), protec\u021bia antibacterian\u0103 (54 cazuri), precum \u0219i epitelizarea spontan\u0103 (46 cazuri).
\u00cen toate cazurile analizate am monitorizat gradul de colonizare bacterian\u0103 (prin examene bacteriologice seriate ale secre\u021biilor din pl\u0103gi), dar \u0219i progresia procesului de dezvoltare al \u021besutului granular \u0219i epiteliului.
Rezultate: Rata zilnic\u0103 a procesului de vindecare a oscilat \u00eentre 3.22% din suprafa\u021ba ini\u021bial\u0103 a pl\u0103gii (\u00een cazul arsurilor) \u0219i 2.03% din suprafa\u021ba ini\u021bial\u0103 a pl\u0103gii (\u00een cazul pl\u0103gilor cronice de alt\u0103 etiologie).
Concluzii: Am constatat c\u0103 exist\u0103 corela\u021bii importante \u00eentre viteza de progresie a vindec\u0103rii \u0219i terapia local\u0103 aplicat\u0103: debridare enzimatic\u0103 lent\u0103 sau prin detersie umed\u0103, terapie cu presiune negativ\u0103 sau debridare excizional\u0103, topice antibacteriene cu sau f\u0103r\u0103 con\u021binut \u00een acid hialuronic. Utilizarea topicelor cu acid hialuronic \u0219i a pansamentelor inteligente individualizate pot influien\u021ba \u00een mod eficient procesul de vindecare.<\/p>\n

Material \u0219i metod\u0103: Am analizat un num\u0103r de 64 de cazuri (31 femei) cu pl\u0103gi cronice de diverse etiologii (32 arsuri vechi, neglijate terapeutic 5-7 zile postcombustional; 14 pl\u0103gi chirurgicale complicate, cu evolu\u021bie trenant\u0103; 18 leziuni trofice- ulcere venoase\/arteriale, escare de decubit, ulcera\u021bii Marjolin postiradiere, ulcera\u021bii pe picior diabetic) tratate \u00eentr-o perioad\u0103 de timp de 7 ani. Pentru tratamentul acestor pl\u0103gi am aplicat protocoale terapeutice generale \u0219i locale.
Tratamentul local a urmat protocoale diferite care s\u0103 asigure detersia pl\u0103gii (42 cazuri), \u00eendep\u0103rtarea biofilmului (26 cazuri), stimularea dezvolt\u0103rii re\u021belei vasculare \u0219i \u021besutului granular (42 cazuri), protec\u021bia antibacterian\u0103 (54 cazuri), precum \u0219i epitelizarea spontan\u0103 (46 cazuri).
\u00cen toate cazurile analizate am monitorizat gradul de colonizare bacterian\u0103 (prin examene bacteriologice seriate ale secre\u021biilor din pl\u0103gi), dar \u0219i progresia procesului de dezvoltare al \u021besutului granular \u0219i epiteliului.
Rezultate: Rata zilnic\u0103 a procesului de vindecare a oscilat \u00eentre 3.22% din suprafa\u021ba ini\u021bial\u0103 a pl\u0103gii (\u00een cazul arsurilor) \u0219i 2.03% din suprafa\u021ba ini\u021bial\u0103 a pl\u0103gii (\u00een cazul pl\u0103gilor cronice de alt\u0103 etiologie).

Matherial and method: We analyzed a number of 64 cases (31 women) with chronic wounds of various etiologies (32 old burns, therapeutically neglected 5-7 days post-combustion ; 14 complicated surgical wounds with long evolution; 18 trophic lesions- venous\/arterial ulcers, decubitus bedsores, post-irradiation Marjolin ulceration, diabetic foot ulcers) treated in a period of time of 7 years. For the treatment of these wounds we have applied general and local therapeutic protocols. Local treatment followed different protocols to ensure wound detersion (42 cases), biofilm removal (26 cases), local vascular network faster development and granular tissue growth (42 cases), antibacterial protection (54 cases), as well as spontaneous epithelialization (46 cases).
In all the cases, we monitored the degree of bacterial colonization (through serial bacteriological examinations of wound secretions), the progression of the granular tissue and epithelium development.
Results: The daily rate of the healing process oscillated between values of 3.22% of the initial wound surface (in the case of burns) and 2.03% of the initial wound surface (in the case of chronic wounds of another etiology).
Conclusions: We discovered important correlations between the rate of healing process progression and the applied local therapy: delayed enzymatic debridement or wet detersion, negative pressure wound therapy or excisional debridement, antibacterial topical with or without hyaluronic acid content.
The use of hyaluronic acid topical and individualized intelligent dressings can effectively influence the wound healing, can reduce the number of days of medical care, but also of surgical sessions, as well as the need for general therapy with antibiotics.<\/p>\n
