Din punct de vedere genetic este de remarcat c\u0103 genele asociate LES sunt majoritatea \u00een regiuni necodante. De\u0219i LES este considerat cea mai important\u0103 boal\u0103 autoimun\u0103, numeroase mecanisme care \u021bin de sistemul imun \u00eenn\u0103scut au fost puse \u00een eviden\u021b\u0103 recent cum ar fi rolul TLR, rolul inteferonului \u0219i semn\u0103turii interferonului, rolul IL-17, IL-8 etc. Deoarece o nevoie neacoperit\u0103 \u0219i semnificativ\u0103 \u00een LES este diagnosticul precoce, au fost dezvoltate clinici de LES precoce \u0219i se caut\u0103 modalit\u0103\u021bi de a reduce timpul de la apari\u021bia primelor simptome p\u00e2n\u0103 la diagnosticul cert de LES prin noi criterii de clasificare, noi biomarkeri \u0219i eventual utilizarea inteligen\u021bei artificiale. Sunt necesare de asemenea schimb\u0103ri \u00een atitudinea terapeutic\u0103 \u00een primul r\u00e2nd prin utilizarea consecvent\u0103 a Hidroxiclorochinei \u0219i prin reducerea expunerii pe termen lung la glucocorticoizi. Reducerea daunelor produse de boal\u0103 dar \u0219i de tratamentul prelungit cu glucocorticoizi se poate face prin utilizarea terapiilor imunosupresoare conven\u021bionale (exist\u0103 medica\u021bii noi introduse \u00een arsenalul terapeutic) dar \u0219i biologice inclusiv asocieri sau secven\u021be de biologice pentru formele severe de LES.

\nUniversity of Medicine and Pharmacy \u201cCarol Davila\u201d Bucharest, Romania<\/p>\nA large volume of new and challenging data concerning pathogenesis, evaluation and treatment of systemic lupus erythematosus (SLE) was published in the last years. A particular genetic aspect is that the majority of the susceptibility genes for SLE are located in non-coding regions. Even if SLE is considered the most important systemic autoimmune disease, recent studies highlighted the role of innate immune system components as TLR, interferons and interferon signature, IL-17, IL-8 and other. Early diagnosis is still a significant unmet need in SLE, early SLE clinics are developed and they are trying to reduce the delay between the first symptoms to the positive diagnosis of SLE by using new biomarkers, new classification criteria and possible AI. The management of SLE should be modified to include constantly Hydroxychloroquine but should also reduce long-term exposure to glucocorticoids. It is possible to reduce the damages produced by the disease itself but also by glucocorticoids by using immunosuppressive treatment (including new entries) and biologics or different therapeutic associations or therapeutic sequences for severe forms of SLE.
\nConclusion: Recent published data are showing major changes of diagnosis, monitoring and management of SLE.<\/p>\n

A particular genetic aspect is that the majority of the susceptibility genes for SLE are located in non-coding regions. Even if SLE is considered the most important systemic autoimmune disease, recent studies highlighted the role of innate immune system components as TLR, interferons and interferon signature, IL-17, IL-8 and other. Early diagnosis is still a significant unmet need in SLE, early SLE clinics are developed and they are trying to reduce the delay between the first symptoms to the positive diagnosis of SLE by using new biomarkers, new classification criteria and possible AI. The management of SLE should be modified to include constantly Hydroxychloroquine but should also reduce long-term exposure to glucocorticoids.
