Lupusul eritematos sistemic (LES), boal\u0103 autoimun\u0103 cronic\u0103 cu afectare multisistemic\u0103, reprezint\u0103 una dintre marile provoc\u0103ri pentru multiplele specialit\u0103\u021bi implicate \u00een managementul acestor pacien\u021bi.Tratamentul cortizonic a reprezentat una dintre primele op\u021biuni de tratament aprobate pentru aceast\u0103 entitate complex\u0103. De\u0219i utiliza\u021bi de peste 70 de ani, datorit\u0103 eficien\u021bei crescute asupra multiplelor aspecte ale bolii pornind de la formele cutanate p\u00e2n\u0103 la cele amenin\u021b\u0103toare de via\u021b\u0103 dar \u0219i rapidit\u0103\u021bii de instalare a efectului, cortizonicele \u00eenc\u0103 se reg\u0103sesc \u00een ghidurile actuale de tratament. Sunt medicamente ieftine cu utilitate \u00een controlul activit\u0103\u021bii bolii \u0219i pentru prevenirea acutiz\u0103rilor. Nu sunt \u00eens\u0103 ideale pentru pacientul lupic. Acest lucru se datoreaz\u0103 binecunoscutelor lor efecte adverse (toxicitate digestiv\u0103, imunosupresie, promovarea aterosclerozei, osteoporoz\u0103 etc) care, \u00een condi\u021bii de administrare prelungit\u0103 asociat\u0103 cu o doz\u0103 cumulativ\u0103 crescut\u0103, contribuie la dezvoltarea leziunilor de organ (damage) \u0219i la cre\u0219terea mortalit\u0103\u021bii. Este motivul pentru care recomand\u0103rile actuale de tratament stipuleaz\u0103 faptul c\u0103 pe termen mediu \u0219i lung scopul este de men\u021binere a dozei zilnice sub cea de 7.5 mg echivalent de prednison sau, dac\u0103 este posibil, de oprire total\u0103. De altfel, toate medica\u021biile nou aprobate \u00een LES au avut printre obiective \u0219i posibilitatea de sc\u0103dere\/oprire a corticoterapiei.

Systemic lupus erythematosus (SLE), a chronic autoimmune disease with multisystem involvement, represents one of the great challenges for the multiple specialties involved in the management of these patients. Cortisone treatment was one of the first approved options for this complex entity. Although used for more than 70 years, due to the increased effectiveness on multiple aspects of the disease starting from the cutaneous forms to the life-threatening ones and to the rapidity of the therapeutical effect, cortisone is still found in current treatment guidelines. They are inexpensive drugs useful in controlling disease activity and preventing exacerbations. However, they are not ideal for the lupus patient. This is due to their well-known adverse effects (digestive toxicity, immunosuppression, promotion of atherosclerosis, osteoporosis, etc.) which, in conditions of prolonged administration associated with an increased cumulative dose, contribute to the development of organ damage and increased mortality. This is why current treatment recommendations stipulate that in the medium and long term the goal is to keep the daily dose below 7.5 mg prednisone equivalent or, if possible, to stop altogether. In fact, all the newly approved medications in SLE had among their objectives the possibility of decreasing\/stopping cortisone.

Este medic primar medicin\u0103 intern\u0103 \u015fi reumatologie. Din anul 2015, dup\u0103 parcurgerea prin concurs a anterioarelor trepte ierarhice, a devenit conferentiar la aceea\u0219i universitate.
Interesele sale de cercetare includ \u00een special bolile inflamatorii reumatologice artrita reumatoid\u0103, lupusul eritematos sistemic, sindromul Sj\u0151gen \u0219i c\u00e2teva forme de vasculit\u0103.
Este autor \u015fi co-autor a peste 100 de lucr\u0103ri de specialitate publicate \u00een reviste na\u0163ionale \u015fi interna\u0163ionale. A publicat \u015fi coordonat mai multe c\u0103r\u0163i dar \u015fi capitole de carte.
A fost investigator principal \u015fi subinvestigator \u00een studii clinice interna\u021bionale de faz\u0103 a II-a \u0219i a III-a \u00een domeniul reumatologiei \u0219i, de asemenea, coordonatorul rom\u00e2n al unui grant interna\u0163ional Horizon 2020.

Lupusul eritematos sistemic (LES), boal\u0103 autoimun\u0103 cronic\u0103 cu afectare multisistemic\u0103, reprezint\u0103 una dintre marile provoc\u0103ri pentru multiplele specialit\u0103\u021bi implicate \u00een managementul acestor pacien\u021bi.Tratamentul cortizonic a reprezentat una dintre primele op\u021biuni de tratament aprobate pentru aceast\u0103 entitate complex\u0103. De\u0219i utiliza\u021bi de peste 70 de ani, datorit\u0103 eficien\u021bei crescute asupra multiplelor aspecte ale bolii pornind de la formele cutanate p\u00e2n\u0103 la cele amenin\u021b\u0103toare de via\u021b\u0103 dar \u0219i rapidit\u0103\u021bii de instalare a efectului, cortizonicele \u00eenc\u0103 se reg\u0103sesc \u00een ghidurile actuale de tratament. Sunt medicamente ieftine cu utilitate \u00een controlul activit\u0103\u021bii bolii \u0219i pentru prevenirea acutiz\u0103rilor. Nu sunt \u00eens\u0103 ideale pentru pacientul lupic. Acest lucru se datoreaz\u0103 binecunoscutelor lor efecte adverse (toxicitate digestiv\u0103, imunosupresie, promovarea aterosclerozei, osteoporoz\u0103 etc) care, \u00een condi\u021bii de administrare prelungit\u0103 asociat\u0103 cu o doz\u0103 cumulativ\u0103 crescut\u0103, contribuie la dezvoltarea leziunilor de organ (damage) \u0219i la cre\u0219terea mortalit\u0103\u021bii. Este motivul pentru care recomand\u0103rile actuale de tratament stipuleaz\u0103 faptul c\u0103 pe termen mediu \u0219i lung scopul este de men\u021binere a dozei zilnice sub cea de 7.5 mg echivalent de prednison sau, dac\u0103 este posibil, de oprire total\u0103. De altfel, toate medica\u021biile nou aprobate \u00een LES au avut printre obiective \u0219i posibilitatea de sc\u0103dere\/oprire a corticoterapiei.

Old and new trends in the use of glucocorticoids in patients with systemic lupus erythematos<\/span><\/p>\n Keywords: lupus, cortison, glucocorticoids, treatment<\/p>\n Systemic lupus erythematosus (SLE), a chronic autoimmune disease with multisystem involvement, represents one of the great challenges for the multiple specialties involved in the management of these patients. Cortisone treatment was one of the first approved options for this complex entity. Although used for more than 70 years, due to the increased effectiveness on multiple aspects of the disease starting from the cutaneous forms to the life-threatening ones and to the rapidity of the therapeutical effect, cortisone is still found in current treatment guidelines. They are inexpensive drugs useful in controlling disease activity and preventing exacerbations. However, they are not ideal for the lupus patient. This is due to their well-known adverse effects (digestive toxicity, immunosuppression, promotion of atherosclerosis, osteoporosis, etc.) which, in conditions of prolonged administration associated with an increased cumulative dose, contribute to the development of organ damage and increased mortality. This is why current treatment recommendations stipulate that in the medium and long term the goal is to keep the daily dose below 7.5 mg prednisone equivalent or, if possible, to stop altogether. In fact, all the newly approved medications in SLE had among their objectives the possibility of decreasing\/stopping cortisone.
