I Found about Demodex! How do I treat it?<\/span>
Author: Camelia-Margareta Bogd\u0103nici<\/span><\/p>\nDemodex can spread from the face to the eyelids, probably leading to blepharitis and also rosacea, which may be the link between Dry Eye Disease and Meibomian gland dysfunction. There are two types of Demodex: Demodex folliculorum \u2013 found in eye lash follicles, 0.3-0.4 mm long and Demodex brevis \u2013 burrows deep into sebaceous and Meibomian glands. D. brevis is smaller and stubbier (0.2 \u2013 0.3 mm).
Various studies have shown normal rates of colonization that can vary between 20% and 80%, increasing with aging. Both species are found in several populations around the world and different ethnic groups and in regions where the climate is warmer. A more recent studies in Turkey examined superficial skin biopsies from 300 healthy volunteers, revealing a prevalence and density for D. folliculorum and D. brevis of 61.3% (mean 14.3\/cm2) and 23.3% (mean 3.2\/cm2), respectively. Infestation of both D. folliculorum and D. brevis is more common in males than in females, presumably due to greater numbers of sebaceous glands. For patients presenting ocular problems, the prevalence of ocular Demodex detected by eyelash sampling is reported to be between 55% and 70%. In a more general population, the picture is more varied: from 18% to 55%. D. folliculorum is much more common (2.4x) amongst samplet eyelashes than D. brevis. Some factors may change the environment to encourage mite\u2019s proliferation, such as: skin phototype, exposure to sunlight, alcohol intake, smoking, stress, hot beverages, spicy food and abrupt changes in temperature.
Modalities of treatment: natural, essential oil from steamed Melaleuca alternifolia leaves; tea tree or narrow-leaved Paperbark, native to Australia; exhibits antimicrobial, anti-inflammatory, antifungal, and antiviral properties; toxic to Demodex. Eyelid hygiene can be considered essential for promoting and maintaining eyelid health; lid hygiene reduces Demodex counts but does not eradicate the mites; association of a TTO ointment with eyelid hygiene is effective in relieving symptoms of Demodex spp. infestation. The treatment of Demodex blepharitis needs to be comprehensive, directed at eradicating the Demodex mites and managing the signs and symptoms of the associated blepharitis. Eyelid hygiene is one of the basic principle of Demodex blepharitis to complement medical treatment; the importance of twice daily comprehensive eyelid hygiene procedures with eyelid wipes, warm compress and eyelid massage, in addition to standard therapy of Demodex blepharitis was demonstrated.
Conclusions: Demodex infestation is: very common, especially in older people, and can lead to eyelid and ocular surface diseases such as blepharitis. The mites can directly obstruct follicles and glands and can also be vectors for bacteria, provoking immune responses. Currently, terpinen-4-ol is the most efficient treatment to eradicate mites so far, in addition to a lid hygiene to relieve ocular signs and symptoms.<\/p>\n

Demodex can spread from the face to the eyelids, probably leading to blepharitis and also rosacea, which may be the link between Dry Eye Disease and Meibomian gland dysfunction. There are two types of Demodex: Demodex folliculorum \u2013 found in eye lash follicles, 0.3-0.4 mm long and Demodex brevis \u2013 burrows deep into sebaceous and Meibomian glands. D. brevis is smaller and stubbier (0.2 \u2013 0.3 mm).
Various studies have shown normal rates of colonization that can vary between 20% and 80%, increasing with aging. Both species are found in several populations around the world and different ethnic groups and in regions where the climate is warmer. A more recent studies in Turkey examined superficial skin biopsies from 300 healthy volunteers, revealing a prevalence and density for D. folliculorum and D. brevis of 61.3% (mean 14.3\/cm2) and 23.3% (mean 3.2\/cm2), respectively. Infestation of both D. folliculorum and D. brevis is more common in males than in females, presumably due to greater numbers of sebaceous glands. For patients presenting ocular problems, the prevalence of ocular Demodex detected by eyelash sampling is reported to be between 55% and 70%. In a more general population, the picture is more varied: from 18% to 55%. D. folliculorum is much more common (2.4x) amongst samplet eyelashes than D. brevis. Some factors may change the environment to encourage mite\u2019s proliferation, such as: skin phototype, exposure to sunlight, alcohol intake, smoking, stress, hot beverages, spicy food and abrupt changes in temperature.
Modalities of treatment: natural, essential oil from steamed Melaleuca alternifolia leaves; tea tree or narrow-leaved Paperbark, native to Australia; exhibits antimicrobial, anti-inflammatory, antifungal, and antiviral properties; toxic to Demodex. Eyelid hygiene can be considered essential for promoting and maintaining eyelid health; lid hygiene reduces Demodex counts but does not eradicate the mites; association of a TTO ointment with eyelid hygiene is effective in relieving symptoms of Demodex spp. infestation. The treatment of Demodex blepharitis needs to be comprehensive, directed at eradicating the Demodex mites and managing the signs and symptoms of the associated blepharitis. Eyelid hygiene is one of the basic principle of Demodex blepharitis to complement medical treatment; the importance of twice daily comprehensive eyelid hygiene procedures with eyelid wipes, warm compress and eyelid massage, in addition to standard therapy of Demodex blepharitis was demonstrated.
Conclusions: Demodex infestation is: very common, especially in older people, and can lead to eyelid and ocular surface diseases such as blepharitis. The mites can directly obstruct follicles and glands and can also be vectors for bacteria, provoking immune responses. Currently, terpinen-4-ol is the most efficient treatment to eradicate mites so far, in addition to a lid hygiene to relieve ocular signs and symptoms.<\/p>\n
