Când și cum schimbăm un agent biologic în terapia psoriazisului?

Psoriazisul este o afecțiune inflamatorie sistemică cronică, mediată imun, cu evoluție ondulantă și, uneori, invalidantă. Psoriazisul a beneficiat, în ultimul timp, de o serie de cercetări și descoperiri privind mecanismele fizio-patologice ale bolii, ceea ce a generat posibilitatea unei noi abordări terapeutice. Introducerea agenților biologici a prezentat o contribuție majoră în arsenalul terapeutic al bolii. Totuși, nu toți pacienții prezintă un răspuns favorabil la terapia biologică. Prezența mai multor clase de agenți biologici, cu mecanisme de acțiune fisio-patologice diferite, permit astăzi o particularizare mai bună a terapiei pentru fiecare pacient. Lipsa de răspuns terapeutic la un agent permite, astăzi, schimbarea lui cu un alt agent biologic, cu mecanism de acțiune similar sau total diferit. Astăzi, schimbarea agentului biologic presupune evaluarea complexă a unor factori legați de aspectele farmacologice ale medicamentelor, prezența comorbidităților, perioada de wash-out și unele aspecte privind avantajele și dezavantajele tratamentului din punct de vedere al pacientului.
Din acest punct de vedere încercăm să prezentăm care sunt oportunitățile și recomandările schimbării agentului biologic în tratamentul psoriazisului.

When and How do we change a biological agent in the psoriasis therapy?

Psoriasis is a chronic, immune-mediated systemic inflammatory condition, with an undulating and, sometimes, disabling evolution. Psoriasis has recently benefited from a series of research and discoveries on the pathophysiological mechanisms of the disease, which has generated the possibility of a new therapeutic approach. The introduction of the biological agents has made a major contribution to the therapeutic arsenal of the disease. However, not all patients have a favorable response to the biologic therapy. The presence of several classes of biological agents, with different pathophysiological mechanisms of action, allow today a better customization of the therapy for each patient. The lack of a therapeutic response to an agent allows, nowadays, its exchange with another biological agent, with a similar or totally different mechanism of action. Nowadays, the change of the biological agent involves the complex assessment of the factors related to the pharmacological aspects of the drugs, the presence of comorbidities, the wash-out period and some aspects of the advantages and disadvantages of the treatment from the patient’s point of view.
Therefore, the aim of our paper is to present the opportunities and recommendations for changing the biological agent in the treatment of psoriasis.
